About this file
This is a Past Performance Questionnaire (PPQ) template used by the Bureau of Reclamation – Interior Region 10 (California – Great Basin) for evaluating contractor performance during source selection for solicitation 140R2026R0006.
The questionnaire is designed to be completed by references with direct experience with a contractor and collected as part of the agency's source selection process. Evaluators provide information about the contractor being assessed, including contractor name, contract number, project title, period of performance, dollar value, and evaluator contact information. The form contains nine rating sections: (1) Scope Similarity, assessing how similar the past contract was to the current requirement in terms of size, scope, and complexity; (2) Quality of Product or Service, rated on a five-point scale from Exceptional to Unsatisfactory; (3) Schedule/Timeliness, evaluating the contractor's ability to meet deadlines and milestones; (4) Cost Control, assessing cost management and expense control; (5) Management & Business Relations, rating communication, responsiveness, and problem-solving effectiveness; (6) Regulatory Compliance, confirming adherence to contract terms, safety, environmental, and security requirements; (7) Small Business Utilization, evaluating good faith efforts to meet subcontracting goals where applicable; (8) Overall Performance Rating, providing a final assessment; and (9) a question regarding whether the evaluator would award to the contractor again. Each section includes space for detailed comments. Completed questionnaires should be returned to Charlesetta Seibure, Contract Specialist/Contracting Officer, at Charlesetta_seibure@ios.doi.gov.
