| NOTES: |
| (1) Service Orders prioritize categories are assigned by the Government only |
| (2) Facilities and CDO are the only authorized hospital staff to assign Service Orders after working hours and weekend |
| (3) Provide technical support to other contractors such as making availalbe technical information or perform certain actions to be taken on the systems maintained under this contract (i.e. check and verify certain breakers that interest determined ongoing projects; disconnect and connect fire alarms, smoke detector; research information needed on drawings that other contractors are not aware of, etc.) |
| (4) Service Orders will not be issued for accomplishment of repairs on systems and equipment maintained under Integrated Maintenance Program (IMP). When a problem or a need for repair is identified: |
| - For emergency IMP repairs that affect patient care or deemed urgent or emergency by the hospital staff, the Contractor shall respond to within five (5) minutes of notification, 24 hours a day, seven days a week |
| - For all other IMP repairs, the Contractor shall respond within two (2) hours of notification and complete the repair within 48 hours. |
