2. Objectives
3. Sampling Scheme
4. Inaccessible Areas
The objectives of this survey were to identify, assess, and collect bulk samples of suspect ACM and identify presumed asbestos containing materials (PACM) in accordance with the United States Environmental Protection Agency (U.S. EPA) 40 Code of Federal Regulations (CFR), Part 61, Subpart M,(the National Emission Standard for Hazardous Air Pollutant{NESHAP}), the Occupational Safety and Health Administration (OSHA) Asbestos regulations 29 CFR Part 1926.1101, Construction Industry, and the U.S. EPA 40 CFR Part 763, Asbestos Hazard Emergency Response Act (AHERA).
Specified areas within the building and/or object(s) were surveyed for suspect ACM. Each suspect ACM was assigned a Homogeneous Area(HA) number.
Materials with distinctly separate layers (ex. floor tile and associated tile mastic) were each sampled and designated as separate HAs. Materials with an inseparable layering system (ex. drywall, dryall tape, drywall mud system) are sampled as a composite and designated as one HA. Sample locations were chosen to be representative of the suspect ACM present in the surveyed areas. Destructive and/or invasive sampling techniques may have been employed during this inspection. Each sample ID was designated with an HA number, letters representing the HA type [miscellaneous (MISC), surfacing material (SM), or thermal system insulation (TSI)], and the number of samples collected for each HA (i.e. 001, 002, etc). Sample IDs and sample locations were logged onto a sample chain-of-custody form and the samples were submitted to a laboratory for polorized light microscopy (PLM) or transmission electron microscopy (TEM) analysis.
This survey is not the definitive source for the identification of all ACM/PACM located within the building and/or object(s). Often materials are located in confined or inaccessible locations with little or no visible manifestation of their presence. Common examples are pipes or other utilities located behind chases, ceilings, walls, and materials within electrical fixtures. This survey may be used as a first reference but cannot be looked upon as a comprehensive listing of all ACM/PACM present.
Each material disturbed during renovation or demolition activities should be evaluated on a case-by-case basis in order to determine if the suspect material is included in this survey. This is especially relevant for material previously hidden, inaccessible or obscured. If a suspect material is not described in this survey, the material must be assumed to contain asbestos until subsequent sampling and analysis proves otherwise.
Jacobs/Ewing Cole - A Joint Venture 5 of 280
5. Polarized Light Microscopy (PLM) and Transmission Electron Microspy (TEM)
6. The following material(s) contain asbestos:
Samples collected of suspect ACM were transported to a laboratory for analysis by PLM or TEM. Analysis was performed according to the guidelines of the National Voluntary Laboratory Accreditation Program (NVLAP) administered by the National Institute of Standards and Technology (NIST).
The asbestos laboratory is accredited for PLM and TEM analysis by NVLAP.
The U.S. EPA “Method for the Determination of Asbestos in Bulk Building Materials,” Method 600/R-93/116, requires that all, multiple, distinct layers must be analyzed individually. Samples with initial PLM results <10% are point counted in order to determine an exact percentage of asbestos content.
The following material contains asbestos:
HA15 - Cementitious pipe elbow insulation (1.75% Chrysotile and 0.25% Crocidolite)
The following materials are assumed to contain asbestos:
HA7 - Metal fire doors (Assumed ACM) (see HA form for specific door locations that are assumed ACM)
Jacobs/Ewing Cole - A Joint Venture 6 of 280
7. Asbestos Recommendations
8. Non-Asbestos Materials Identified
The following materials do not contain asbestos:
HA1 - Tan flexible caulking;
HA2 - Black flexible caulking;
HA3 - White flexible caulking;
HA4 - Tan cove base mastic;
HA5 - 12"x12" vinyl floor tile, white with gray flecks;
HA6 - Black floor mastic;
HA8 - 12"x12" Mauve vinyl floor tile;
HA9 - Black floor mastic under mauve floor tile;
HA10 - 2'x4' white ceiling tiles, wormholes with gouges and pinholes;
HA11 - 2'x4' white ceiling tiles, wormtracks with gouges and pinholes;
HA12 - 1/2" Drywall composite;
HA13 - 5/8" Drywall composite;
HA14 - Cloth insulation wrap;
HA16 - White PVC roofing membrane;
HA17 - 1/2" white Densdeck sheeting;
HA18 - Black roofing tar and felt (inseparable);
HA19 - Black soft loose fill material in exhaust vent.
Black foam insulation on air conditioning, heating, and water lines is considered non-suspect material and does not contain asbestos. The Overhead Door on the north end of the building is non-suspect. Clear silicone caulking, located in multiple locations, is a non-suspect material. Fiberboard, or duct board, on the indoor furnaces is non-suspect material. White and pink fiberglass insulation, above the ceiling tiles is non-suspect material. The HVAC trunk lines and ductwork above the ceilings is also non-suspect material; some has aluminum covered fiberglass insulation while other duct is “flex-duct” with fiberglass insulation. All pipe insulation, paper and foil wrap over yellow fiberglass insulation, is non-suspect material (note that this is different from HA14 and HA15). The Poly IsoCyanurate (ISO) foam insulation on the roof is also a non-suspect material and does not contain asbestos.
Any work performed on the above mentioned asbestos material must be performed by properly licensed personnel and in accordance with applicable Federal and State regulations and Navy policy.
Jacobs/Ewing Cole - A Joint Venture 7 of 280
Additional Details:
Objects and items inaccessible and/or outside of scope of the inspection:
1. Rheem Air Conditioning unit located on the exterior of the South end of the building. It is not anticipated that this unit would have any asbestos material in the interior of the unit.
2. Carrier Air conditioning unit located on the exterior of the North end of the building. It is not anticipated that this unit would have any asbestos material in the interior of the unit.
3. Carrier gas furnace located in room 001 (North end). It is not anticipated that this unit would have any asbestos material in the interior of the unit.
4. Goodman gas furnace located in room 004. It is not anticipated that this unit would have any asbestos material in the interior of the unit.
5. All electrical panels, interior and exterior, were “live” and inaccessible.
6. All electrical wiring was fully enclosed in conduit and inaccessible.
7. Water heater inside of room 001 was fully enclosed. The subject water heater is fairly new and assumed not to contain asbestos.
8. There is a drywall ceiling above the lower ceiling tiles; the area between the upper drywall ceiling and the bottom of the roof could not be accessed. It is suspected that there is a void between those two layers with some type of insulating material.
9. Not all interior walls could be penetrated to determine the presence of insulation; the conference room walls, however, were observed to have pink fiberglass insulation, which is a non-suspect material.

Additional Details:
Lead Based Paint (LBP) is paint, varnish, shellac, or other surface coatings that contain lead equal to or greater than 5,000 parts per million (ppm), 0.5% by weight, or 1.0 mg/cm2 (40 CFR 745.103). See summary and results below for determinations. Disturbing paint containing any amount of lead may pose a safety risk.

3. Sampling Scheme
4.
This survey is not the definitive source for the identification of all LBP located within the building and/or objects. Often materials are located in confined or inaccessible locations with little or no visible manifestation of their presence. Common examples are pipes or other utilities located behind chases, ceilings, and materials within electrical fixtures. This survey may be used as a first reference but cannot be utilized as a comprehensive listing of all LBP present. Each material disturbed during renovation or demolition activities should be evaluated on a case-by-case basis in order to determine if the suspect material is included in this inspection.
The objectives of this survey were to identify, assess, and collect samples of potential Lead Based Paint (LBP) in accordance with the United States Consumer Product Safety Commission 16 CFR 1303, Environmental Protection Agency (EPA) 40 CFR 745, the Occupational Safety and Health Administration (OSHA) 29 CFR 1910.1025, General Industry Standard, and OSHA 29 CFR 1926.62, Construction Industry Standard.
Building areas and/or objects were surveyed for suspect LBP. Samples collected from similar building and/or object component areas were chosen to be representative of the suspect LBP present in the building and/or object areas. Each sample in this report is identified with a description that reflects the component, substrate, and color sampled.
In some instances, the MSDS/SDS of the paints used on a given surface can be located and reviewed to determine if it contains lead. If no lead is found in the MSDS/SDS, it will be noted on the sample results table along with the paint description and a copy of the MSDS/SDS will be attached to the report. Samples were collected in accordance with the following method:
CR-PRX11-LBP-FM-05100-000001 01SEP2016
Glazed brick/ceramic, porcelain, mirrors and factory applied finished (FAF) metal components, if present and in good condition, may be selectively tested for lead content on a discretionary basis due to difficulty removing a sufficient amount of paint from the surface for laboratory analysis. FAF building/object components in good condition will be listed in this report and will be assumed to contain LBP, but typically will not be sampled. During renovation activities these items are typically removed as whole components and therefore pose less of a lead hazard than other building/object components.
Inaccessible Areas
ASTM E 1729 “Field Collection of Dried Paint Samples for Lead Determination.” Using this method, paint chip samples were submitted to a laboratory for analysis under chain-of-custody (COC) protocol.

Paint Sample Locations Diagram and/or Photographs - See attached building diagram (s) and/or photos.
Paint chip or dust samples are collected for laboratory analysis by removing one to four square inches of paint from the surface. All layers of paint in the sampled area are included in the sample. Usually samples will contain some of the material beneath the paint, such as wood, plaster, or concrete particles. The amount of this material will be kept to a minimum. Samples collected of suspect LBP are shipped to a lead laboratory for analysis by Flame AAS. The lead laboratory is accredited for Flame AAS analysis by the Environmental Lead Laboratory Accreditation Program (ELLAP). ELLAP is an approved lead laboratory accreditation program under the Environmental Protection Agency (EPA) National Lead Laboratory Accreditation Program (NLLAP). Standard laboratory method EPA SW-846-3050B/7000 is used to quantify lead levels in paint chip sample(s). Sample analysis results are provided for each sample submitted to the laboratory.
If a given material is not described in this survey, the material must be assumed to contain LBP until subsequent sampling and analysis proves otherwise.
Flame Atomic Absorption Spectrophotometry (Flame AAS)
This is especially relevant for materials previously hidden, inaccessible or obscured.
