About this file
This is a Task Assignment Change Order Form (Change Order No. 02) issued by the Florida Department of Environmental Protection (FDEP) for contract CN214 involving the Florida Caverns State Park New Campground and Mountain Bike Trail project. The consultant is George & Associates Consulting Engineers, Inc., with project representation by Brian Miller, PE. The FDEP Contract Manager is Ralph Perkins. The change order was executed on May 19, 2023, with approvals from FDEP budget, project management, and contract authority representatives, as well as the consultant.
This deductive change order modifies cultural resource services work as requested by the Department of State Division of Historic Resources (DHR). Subtask 2b (Case Study Report), originally valued at $5,579.00, is being removed and replaced with Subtask 2d (Archaeological Impacts Summary), valued at $3,254.00. The net reduction to the overall contract is $2,325.00, bringing the total task amount from $82,361.97 to $80,036.97. The change involves removing work on a case study report that would have documented project alternatives and mitigation measures, and substituting it with an archaeological impacts summary letter report describing previously identified archaeological sites within the mountain bike trail footprint and recommending alignment modifications to avoid site impacts. Payment is contingent upon FDEP approval of deliverables for each subtask. The project execution date remains December 2, 2021, with the task end date extended to June 30, 2024.
