About this file
This is a Task Assignment Change Order Form (CN214-TA39-CO1) issued by the Florida Department of Environmental Protection (FDEP) for a consulting services contract with George & Associates, Inc. The change order modifies the existing Task Assignment No. 39 related to boat inspection services at Edward Ball Wakulla Springs State Park, located in Wakulla Springs, Florida. FDEP Contract Manager Jeffrey Whitfield and consultant representative Robert George of George & Associates, Inc. are the primary points of contact for this task assignment.
The change order reduces the original task amount from $9,630.00 by $2,974.67, resulting in a revised total of $6,655.33. The reduction reflects a reallocation of funds as one change order is being added for boat winch testing and certification along with a safety analysis for winch operation. The revised scope includes two subtasks: Subtask 1 (Condition & Valuation Survey) at $5,764.00 and Subtask 2 (Vessel Winch-Hoist Inspection) at $891.33, both on a fixed-price basis with no reimbursable costs. Subtask 2 involves coordination with Ace Industries Crane & Hoist to provide visual inspection of the motorized winch system, rail, and trolley components, with deliverables including a comprehensive inspection report with picture-integrated deficiency reporting and a Job Hazard Analysis. The task execution period runs from April 13, 2022, through December 30, 2022, with no changes to the task end date. All required FDEP approvals were obtained and digitally signed as of April 27-28, 2022.
