Summary of Attachment A9 – Adult and Older Adult Culturally Appropriate Outpatient Continuum Services

This scope of work document outlines a behavioral health services contract opportunity issued by the Santa Clara County Behavioral Health Services Department (BHSD) under RFSQ-BHS-FY23-0210 for Category 2: Outpatient Services. The County seeks qualified offerors to design and operate comprehensive outpatient mental health and substance use services for adults ages 18-59 and older adults ages 60 and over with severe mental illness and/or co-occurring substance use disorders. The program will serve previously unserved and underserved populations across four county regions: North County (Palo Alto, Mountain View, Sunnyvale, Los Altos areas), Central County (San Jose, Cupertino, Santa Clara, Los Gatos, Saratoga, Campbell), East County (San Jose Foothills, Berryessa, Milpitas), and South County (Gilroy, Morgan Hill, San Martin). Services must include culturally appropriate and trauma-informed outpatient care, individual and family counseling, case management, medication management, crisis intervention available 24/7, and linkage to community resources. The offeror must maintain Medi-Cal and Medicare certification with a certified service delivery site operating Monday through Friday during regular office hours (minimum 40 hours per week) with after-hours and weekend services available as needed.

The scope of work requires the offeror to establish an interdisciplinary team including psychiatrists, licensed clinical counselors, social workers, psychiatric nurses, peer mentors, and bilingual staff capable of serving Santa Clara County's threshold languages (Spanish, Vietnamese, Mandarin, Cantonese, Tagalog, Farsi, and Russian). Staffing must comply with BHSD co-occurring standards, reflective supervision requirements, and diversity and inclusion training. The program must implement the County's myAvatar electronic health record system and submit quarterly MHSA reporting data including client demographics, CSI system data, and outcomes measurements using clinician-completed assessments (including Illness Management and Recovery Scale), client-completed surveys (MHSIP Consumer Survey, Client Satisfaction Survey, and cultural competence assessments), and transition and discharge tracking. Key outcome benchmarks include improving access and engagement for culturally diverse clients, successfully transitioning clients to lower levels of care, decreasing emergency psychiatric services utilization, and increasing client satisfaction. Referrals are accepted from the BHSD Call Center with urgent referrals to be triaged within 24 hours and seen within five business days, and the program must develop discharge and transition plans based on client progress and level of care needs.
