About this file
This is an evaluation criteria appendix for a Request for Service Quotation (RFSQ) issued by Santa Clara County's Behavioral Health Services Department (BHSD) for Adult and Older Adult Assertive Community Treatment (ACT) services as part of a Full Service Partnership program. The document outlines the scoring rubric and qualifications required for organizations seeking to provide comprehensive ACT services to adults ages 18 through 59 and older adults ages 60 and over, including specialized services for LGBTQ+ communities, individuals with co-occurring conditions, justice-involved populations, and those with severe mental illness. Offerors must demonstrate organizational experience serving diverse populations across all geographic regions in Santa Clara County and meet Medi-Cal requirements including certification, documentation, staff credentialing, billing requirements, and CalAIM behavioral health standards.
Evaluation of proposals will be conducted across five sections totaling 100 points: Organizational Structure and Experience (70 points), Proposed Program Description (15 points), Historical Performance Analysis (5 points), Electronic Health Record Capacity (5 points), and Financial Solvency (5 points). Key requirements include evidence of evidence-based and trauma-informed treatment practices, comprehensive staffing plans with personnel retention strategies, 24/7 service availability and crisis support, fidelity to the Dartmouth Assertive Community Treatment Scale, cultural competency across County threshold languages, and demonstrated collaborations with County departments, housing organizations, and the justice system. Organizations must also provide emergency and disaster service delivery plans, data collection and outcome evaluation processes compliant with State Data Collection and Reporting systems, and documentation of current financial stability and organizational viability.
