About this file
This is Addendum #1 to an Electronic Request for Statement of Qualifications (RFSQ-BHS-FY23-0210) issued by the County of Santa Clara's Behavioral Health Services Division (BHSD) on June 29, 2023, regarding Adult and Older Adult System of Care Services. The addendum modifies submission requirements and documentation procedures for the original RFSQ. Current County Contracted Providers (CCPs) actively providing any of the listed services are subject to automatic qualification of existing programs; however, these providers must submit Sections I and III of Appendix A along with financial stability documentation to renew existing programs. CCPs seeking to provide additional programs or services not included in their current contracts must submit all required documents as specified in the RFSQ. The County reserves the right to contract with qualified offerors as needs are identified and may maintain each Qualified List for five years.
The addendum establishes specific submission procedures requiring organizations to submit materials in a consolidated email format to BHSD.Solicitations@hhs.sccgov.org, organizing responses into numbered attachments including appendices, audited financial statements, and program-specific responses. Written responses are limited to five pages per program, excluding organizational charts and resumes. The addendum lists 21 programs within the system of care, including In-Home Outreach Teams, Wellness Centers, Outpatient Continuum Services, Full Service Partnerships, Assertive Community Treatment, and Housing Programs, with multiple current providers identified for each service category. The list of current providers demonstrates an incumbent-based system with established organizations such as Bill Wilson Center, Gardner Family Health Network, Community Solutions, and Mental Health Systems currently contracted to deliver these services across the Santa Clara County adult and older adult behavioral health system.
