About this file
This is Addendum 7 to an Electronic Request for Statement of Qualifications (RFSQ) issued by the County of Santa Clara for Adult and Older Adult System of Care Services (RFSQBHSFY230210), dated November 18, 2025. The addendum updates program nomenclature and revises multiple appendices and attachments to align service offerings with updated naming conventions. Key program updates include renaming A5 and B5 to "Culturally Appropriate Wellness Centers" and A9 and B9 to "Adult and Older Adult Culturally Appropriate Outpatient Continuum Services." Additionally, Category 6 has been expanded to include A22: Adult and Older Adult Substance Use Treatment Outpatient Continuum Services, covering ASAM Level 1 and ASAM Level 2.1 services. Multiple documents have been revised in their entirety, including Appendix B9, Appendix G (Cultural and Linguistic Competence), and Attachments A5 and A9 related to scopes of work.
Attachment A22 for Adult and Older Adult Substance Use Treatment Outpatient Continuum Services has been updated to specify requirements for ASAM Level 2.1 group-based, nonresidential intensive structured interventions. Intensive Outpatient Treatment Services must be provided when medically necessary with programming intensity ranging from a minimum of nine to a maximum of nineteen hours weekly for adults, and six to nineteen hours weekly for members under twenty-one years of age. Services may exceed the maximum hours based on individual medical necessity. All other specifications, terms, and conditions of the original RFSQ remain unchanged.
