About this file
This document is Addendum #2 to the Electronic Request for Statement of Qualifications (RFSQ-BHS-FY23-0210) issued by the County of Santa Clara, California for Adult and Older Adult System of Care Services. The addendum, issued on July 18, 2023, incorporates multiple revisions to the original RFSQ and its appendices, including updates to service categories, financial documentation requirements, and appendix materials. The revisions consolidate Individual Placement and Support and Supported Employment services into a single merged optional level of care across multiple appendices. A pre-proposal conference was held on July 12, 2023, with questions and answers documented in this addendum. Current County Contracted Providers may auto-qualify for existing services by submitting completed Sections I and III of Appendix A along with financial stability documentation, or may submit complete proposals for new services not currently under contract. New program providers would commence services in Fiscal Year 2025.
Financial requirements for all offerors include submission of audited financial statements covering the last two fiscal years with applicable notes and independent auditor reports, or alternative documentation such as Dun & Bradstreet reports demonstrating sufficient cash or credit reserves. Contracted rates for Fiscal Year 2024 are specified by staff position and range from $245.27 per hour for Licensed Psychiatric Technicians to $1,110.80 per hour for Psychiatrists, with outreach services billed at $3.48 per minute. For Full Service Partnership programs in Category 3, rates are ten percent higher. Budget submissions for new programs must include estimates for both Fiscal Year 2024 and 2025, utilizing a 50/50 revenue split between Medi-Cal and County General Fund for estimation purposes. The BHSD Finance team will finalize rates and funding allocations during subsequent contract negotiations.

J. Q&A from July 12, 2023 Pre-Proposal Conference
Disclaimer: Similar or identical questions have been merged for concision.
1. Q: Can the solicitations lead share the thinking behind submitting renewals and new proposals together vs. asking for new proposals separately?
A: As discussed at the Fiscal Year (FY) 2023 BHSD Community Convening meeting related to BHSD solicitations, the intent of this RFSQ is to streamline the qualification process for current County Contracted Providers that have already successfully completed a competitive solicitation and negotiation. This RFSQ is designed so Offerors may auto-qualify for the current services that their agency is contracted with BHSD for, and/or submit responses to provide new Adult and Older Adult services (not included in an existing contract with the BHSD). The requirement for Offers to submit both auto-qualification and offer for new services currently not contracted for as part of a singular response (per Offeror) reduces redundancies in the “qualification” process and the need for additional manpower on both County BHSD Administration and County Contracted Provider (CCP) submission teams.
2. Q: Do Offerors who are interested in providing multiple services that support the Adult and Older Adult System of Care need to provide one (1) set of documents per organization with multiple written responses?
A: Yes, that is correct. Offerors interested in providing any combination of existing and/or new services currently not contracted for shall provide one (1) set of Appendices and responses for new programs (e.g., budget narrative, Appendix Bx, and written responses) in accordance with the RFSQ, Section IV.B.1.
3. Q: Question 43 on Periscope Holdings indicated that BHSD has released Addendum #2, but no documents are available. Please advise.
A: Thank you for bringing this to our attention; this document is Addendum #2.
4. Q: In Appendix A, “Coversheet”, there are two signatory lines. Please advise if Offerors will need two different signatories if the CEO signs, given there are two signatory lines.
A: There are two signatory lines available in the event they are needed; otherwise, a sole CEO signatory (or any authorized signatory for an organization) is acceptable.
5. Q: Appendix A, “Coversheet references County terms and conditions for interested Offerors - where can Offerors reference this information?
A: Refer to the RFSQ Section III.A.14 for County Rights.
6. Q: For providers that wish to maintain and not change or expand services, Offerors are to submit Appendix A, completing Sections I & III, and submit organizational audited financial statements. The Q&A response on Periscope is clear, but can the solicitations lead confirm again that this is sufficient?
A: Yes, confirming the submission of the referenced documents are sufficient.
7. Q: Where can I find Appendix D?
A: All documents in the RFSQ packet being presented at this Pre-proposal Conference may be downloaded via Periscope Holdings:
https://www.bidsync.com/bidsync-app-web/vendor/links/BidDetail.xhtml?bidid=2111106&vendorview=Y https://www.bidsync.com/bidsync-app-web/vendor/links/BidDetail.xhtml?bidid=2111106&vendorview=Y https://www.bidsync.com/bidsync-app-web/vendor/links/BidDetail.xhtml?bidid=2111106&vendorview=Y
8. Q: There is a typo in Section IV.E. Table 14 for Appendix G, “Cultural Competence Form” and it is referred as Appendix H. Will this be relabeled?
A: Thank you, this was also previously brought to our attention and will be relabeled and reposted on Periscope Holdings as part of Addendum #2.
9. Q: If an Offeror plans to submit a combination mix of (1) auto-qualification and (2) offer to provide services that the agency is not currently contracted for, should Offeror only submit budget(s) for each new service?
A: Correct, the BHSD requires one (1) budget submission per new service/program.
Per RFSQ Section I.A.3, if auto-qualification is requested, the process for current BHSD County Contracted Providers (CCPs) that provide any of the services listed in this RFSQ are subject to automatic qualification of existing programs following the completion and submission of:
• Sections I and III of the Appendix A; and
• Financial stability/solvency documentation (as referenced in Section IV.K.1 of the RFSQ).
Regardless of application for new and/or existing programs, one (1) copy of both Appendix A and Financial stability/solvency documentation are required.
If any CCPs have an interest in providing additional services listed in the RFSQ that are not included in their organization’s current contract with BHSD, all documents as required by the RFSQ must be completed and submitted per Section IV.D.4.
10. Q: On page 5 of Addendum #1, it states that Community Solutions has the Post- Release (PRCS) program and Gardner Health Services has the Aftercare Program.
Please correct this to reflect that Gardner has the PRCS program and Community Solutions has the Aftercare Program.
A: Thank you, this was also previously brought to our attention. This Addendum #2 includes the revision and replacement of Addendum #1, Section C, “Current Program County Contracted Providers”.
11. Q: Will the County provide a guidance to Offerors interested in providing new services currently not contracted for regarding ability to address the scope of work (SOW)? The SOW is prescriptive. Does the County require confirmation of ability or is more detail on Offeror ability needed?
For example, in any given Scope of Work – how shall Offerors address every single component in the proposal with a five (5) page limit?
A: Per Section IV.E.2, any proposals for new programs should include references to the Miscellaneous Supporting Documents, which can contain any form of written media (e.g., brochures, policies, data sets). For example, a proposal may state “please see pages # - # in the Miscellaneous documents”.
12. Q: For new program submissions, does the BHSD require both Fiscal Year (FY) 2024 and FY 2025 budgets? The RFSQ states new program providers would start in FY 2025, two years from now.
A: Yes, confirming that Appendix I (per new program) shall include sheets and submissions for both FY 2024 and 2025.
13. Q: What are the revenue splits for each program as the breakdowns are needed to complete Appendix I, “Budget Narrative Template”?
A: For the purposes of creating an estimate for the County to review, create estimates using a 50/50 split (Medi-Cal and County General Fund).
14. Q: Regarding Appendix I, “Budget Narrative Template”: how should Offerors determine the revenue sources and percentage of funding source split for new programs? Including County General Fund, how should Offerors determine allocations via CGF or other funding sources?
A: Add rates per service and multiply units for a calculation of monthly estimates.
Insert data in bright blue areas marked with “0” as a placeholder as applicable.
Rates submitted shall exclude County General Fund and include Medi-Cal reimbursable costs. The BHSD Finance team shall continue to collaborate with QPs on final rates during contract negotiations.
15. Q: Will the caseload split between Medi-Cal and Non Medi-Cal also be 50/50? Or will we need to determine this internally?
A: To clarify, clients served shall be reflective of BHSD needs and Offeror capacity.
Finalized budgets, funding sources, estimated clients served, and additional information on reimbursement percentages will be determined upon contract negotiations.
16. Q: Do current BHSD County Contracted Providers (CCPs) actively contracting with the BHSD to provide services listed in this RFSQ need to reapply if they have minor changes and only interested in expanding the population or specialties?
A: Minor changes within existing programs (e.g., program expansion plans, total clients served, etc.) shall still allow current BHSD CCPs contracted for services under this RFSQ to qualify each program for the auto-qualification process.
Those minor changes would be discussed during the contract negotiations meeting(s).
17. Q: Do existing providers need to submit Appendix E, “Declaration Submittal Form” for the auto qualification? Appendix A Section III.D indicates we already signed a non-collusion affidavit.
A: No, BHSD confirms that Appendix E shall be waived for existing providers seeking auto-qualification.
18. Q: Since [our organization] is an existing provider, we won’t need to do the entire proposal - correct?
A: Please note that there are two (2) submission options for existing providers in regard to this RFSQ.
If Offerors plan to submit for a combination of existing and new services currently not contracted for, all documents will be needed.
