About this file
This is Addendum #4 to an Electronic Request for Statement of Qualifications (RFSQ-BHS-FY23-0210) issued by the County of Santa Clara for Adult and Older Adult System of Care Services. The addendum, issued on April 3, 2024, revises three key components of the original solicitation: the Periscope Q&A deadline, Appendix H regarding Electronic Health Record Status, and Attachment A18 for the Assisted Outpatient Treatment (AOT) program. The addendum addresses formatting issues in Attachment A18 and provides clarifications on program requirements and target population criteria.
The revised Attachment A18 specifies that the AOT program shall serve adults aged 18-59 and older adults aged 60 and over with severe and persistent mental health and/or co-occurring conditions resulting in substantial functional impairments. Eligible clients must have Medi-Cal benefits designated to Santa Clara County, be uninsured, or meet other eligibility criteria as defined by Behavioral Health Services Division policy. Clients must meet specific clinical criteria including severe mental illness, demonstrated need for AOT services, historical lack of compliance with voluntary treatment, and a hospitalization history or violent behavior within defined timeframes. The Q&A deadline in Periscope has been extended to March 1, 2028 at 3:00 p.m. Pacific Time. All other specifications, terms, and conditions of the original RFSQ remain unchanged.
