About this file

This document is a limited asbestos good faith survey report for the Mann-Grandstaff VA Medical Center located in Spokane, Washington. The survey was conducted to identify asbestos-containing building materials that could be impacted by renovation of elevators and surrounding areas in Buildings 1 and 27. Asbestos-containing materials identified include floor tiles and associated black mastic on various floors of Building 1, as well as vinyl sheet flooring and associated black mastic on the sixth floor. The report provides sample locations, quantities, and asbestos content test results. It will help inform renovation work under federal contract number 36C26024R0009 between the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 20 and contractors to refurbish and upgrade elevators in Buildings 1 and 27 at the medical center.

1.0 INTRODUCTION

Kyron Environmental Inc. (Kyron) was contracted by Spees Design Build to conduct a limited Asbestos Good Faith Survey at the Mann-Grandstaff VA Medical Center (VAMC) located at 4815 N. Assembly Street, Spokane, WA 99205. The survey was conducted in accordance with EPA protocol outlined in 40 CFR Part 763. The intent of the survey was to identify asbestos-containing building materials (ACBM) that could be impacted by renovation activities of the elevators and surrounding areas at the VAMC. The inspection and sampling of suspect Asbestos-Containing Material (ACM) was conducted on February 8, 2021 by accredited Asbestos Hazard Emergency Response Act (AHERA) Building Inspectors Tim Lee and Graham Langley. Please see attached AHERA Building Inspector Certificates.

ACM was identified or assumed during this limited asbestos survey.

• All building materials on 5th Floor of Building 1 are assumed ACM

• Various Floor Tile & Associated Black Mastic

• Vinyl Sheet Flooring & Associated Black Mastic

3.0 LIMITED ASBESTOS SURVEY SCOPE OF WORK

The objective of the limited asbestos survey was to determine if asbestos-containing materials (ACM) were present that could be impacted during the renovation project at the VAMC. Areas undergoing renovation include limited areas of lobbies near elevators in the main Building 1 basement and Floors 1 through 8, mechanical room of 9th & 10th Floors, as well as elevator areas and lobbies in Building 27, basement, 1st and 2nd Floors. Kyron collected bulk samples of suspect materials in accordance with EPA protocol, including destructive sampling techniques in accordance with the criteria outlined in 40 CFR

763.85 and 763.86: Environmental Protection Agency’s (EPA) interpretations and guidelines of the Asbestos Hazard Emergency Response Act (AHERA). The following suspect materials were identified and/or sampled for asbestos content. Maps identifying sample locations is attached to this report.

Asbestos inspection and sampling methods were conducted in accordance with criteria outlined in 40 CFR 763.85 and 763.86. Samples were submitted to Mountain Laboratories located at 9922 East Montgomery Avenue, Suite 13, Spokane Valley, WA 99206, NVLAP Code 101890-0, and analyzed using Polarized Light Microscopy (PLM) method EPA 600/R-93-116. Chain of Custody and analytical results are attached to this report.

4.0 ASBESTOS-CONTAINING MATERIALS (ACM)

An asbestos containing material (ACM) is defined by EPA’s National Emissions Standard for Hazardous Air Pollutants (NESHAP) as any material containing more than one percent (>1%) asbestos. The following table lists materials that were sampled and/or identified as ACM for the scope of work in this limited asbestos good faith survey. Please refer to attached maps for sample locations.

5.0 ASSUMED ASBESTOS-CONTAINING MATERIALS (ASSUMED ACM)

The 5th Floor of the VAMC Building 1 was unable to be accessed due to surgery protocols in place at the VAMC. Therefore, all building materials are assumed to be ACM until such time that these materials can be sampled to determine asbestos content.

All elevator components, building envelope electrical devices and components in the structure were in operation, therefore, they could not be inspected to determine if considered suspect ACBM. It is recommended that if these materials are planned to be disturbed, a qualified electrician should lock-out/tag-out electricity prior to inspection to determine if suspect materials are present and sampling is needed or if materials will be assumed as ACM.

6.0 NON-ASBESTOS-CONTAINING MATERIALS

Wood, metal, glass, fiberglass, silicone, and rubber were determined by the AHERA Building Inspector's expertise to be non-suspect ACBM, therefore these materials were not sampled and are considered to be non-ACM. The following table lists sample number, material descriptions, homogenous areas, and map locations of materials sampled and determined as non-asbestos-containing materials (non-ACM). Please refer to attached maps for sample locations.

7.0 RECOMMENDATIONS

Building owners are responsible under Washington Department of Labor and Industries Division of Occupational Safety & Health (DOSH) regulations to notify all employees and contractors working on renovation or demolition projects of the presence or absence of asbestos containing materials. It is required that a summary of the results of the asbestos survey be posted, either by the property owner or the owner's agent (e.g., general contractor) at the work site. This report should be communicated in writing to all persons who may come into contact with the materials. A signed acknowledgment by a responsible party (owner or owner's representative) should be included in or attached to the report. See Attachment E for acknowledgment form, “Communicating Survey Findings to Affected Parties”.

IMPORTANT: If a suspect asbestos-containing building material not identified as ACM or non-ACM in this report is discovered that may be impacted during this project, it must be sampled for asbestos content by an AHERA Asbestos Building Inspector or assumed to be an asbestos containing material (ACM).

8.0 LIMITING CONDITIONS

The recommendations present in this report were prepared following applicable asbestos regulations and generally accepted enforcement protocol in the State of Washington for the described property only at the time the inspection was performed.
