About this file
This is Addendum #4 to a construction project for Kent Memorial Library issued by Wyeth Architects on November 24, 2025. The project involves renovation and construction work at the library facility, with specific focus on structural elements including CMU walls, elevator shaft construction with beam pockets, reinforcement requirements, and interior finishes. The addendum addresses architectural drawings (revision 2), clarifies material specifications, and confirms project requirements including ADA-compliant signage for multiple rooms and restrooms. Key dates include a bid due date of December 5, 2025 at 4:00 p.m. The library will be fully vacated during the construction period, allowing contractors complete site access. Contractors are required to provide onsite office space, which may be furnished through a trailer or other means, and must handle all site logistics including material delivery and debris removal at their discretion.
The addendum clarifies several bidding requirements and procurement details, including confirmation that no bid bond is required (bidders may enter 0%), that a DAS Contractor Prequalification Certificate alone is acceptable without supplementary documentation, and that an ACORD Certificate of Insurance is acceptable in lieu of AIA G705. The project does not require LEED certification but requests documentation of sustainable materials through EPDs, PHDs, or equivalent submittals. No irrigation, designated fire alarm vendor, or window substitutions during bidding are required. Questions and answers provided in the addendum address specifications gaps, clarify drawing conflicts between sheets A1.1B and S1.1 for CMU wall layouts, and provide technical details for beam pocket installation and reinforcement requirements referenced in specification sections S2.1, S2.2, and S3.1.

Questions From Bidders:
Q1: CMU wall | Elevator shaft + partial Rm128 wall | DWG A1.1B and S1.1
CMU wall layout is not same, please confirm which one to use for pricing.
A: Use A1.1B where there are CMU walls between the elevator shaft and the wall with door 125.
Q2: Are there Beam Pockets required inside the of Elevator shaft? Please
Clarify.
A: Yes. W-8 beam pockets, bearing plates, and suitable reinforcement for the elevator hoist beam as required for selected product. See E1, E3, and E5 on S2.2 for typical beam pocket details.
Q3. CMU and rebar are present in this project, but there appears to be no correlating specifications. Please provide specifications for CMU and rebar/reinforcing required.
A: See S2.1, S3.1 for rebar notes. See the note in detail G1/S3.1 for elevator shaft reinforcing.
Q4: Q10 of Addendum 2 states a bid bond is not required and to carry 0%, however, Q15 states a bid bond is required and to refer to Q10. Please confirm a bid bond is not required.
A: A bid bond is not required. You may enter “0%” on the bid form.
Apologies for the typo in Q15. Thank you for catching it.
Q5: Clarify the questions and answers provided in Addendum #2.
Q10: States no bid bond required. Use 0%.
Q15. States that a bid bond is required. See question #10.
So, my question is: Is a bid form required but the penal sum is listed as 0% or is one not required?
A: See answer to Q4 above.
Q6: On pg.3 of the 00 04 00 bid form required attachments, line 1 calls for a
DAS Contractor Prequalification Certificate and refers to Section 00 11 16.
Pg.5 article 12 of this section states that either an AIA A305-2020 or CT DAS Contractor Prequalification Forms are required. Is the DAS certificate alone acceptable or is supplementary documentation required?
A: The DAS certificate alone is acceptable.
Q7: Section 01 50 00 page 4 states contractors will need to provide a modem, computer, and printer/plotter for the onsite office. Is this a non-negotiable requirement? This will add a decent cost to general conditions and we have in-house ability to print and deliver documents onsite.
A: As long as you can cover the needs somehow, that’s sufficient.
Q8: Section 01 81 13 illustrates sustainable design documentation required for
LEED submittals. What version of LEED is applicable to this project and to what extent is LEED documentation required for submittals, as it is not mentioned in every division.
A: We are not seeking a sustainable building standard certification. We are looking for documentation that materials are sustainable. If they exist, provide EPDs, PHDs, or some form of LEED or ILFI and/or multi-attribute submittals.
Q9: Article 16 of the Supplementary General Conditions instructs contractors to submit a ‘G705 Certificate of Insurance’. AIA G705 is for the list of subcontractors; what AIA form is required here or is an ACORD Certificate of Insurance acceptable?
A: An ACORD certificate is acceptable.
Q10: Can you confirm if irrigation is needed for this project? We went through the bid documents but we only see some mentioning of irrigation in the specs and there are no planting plans or landscaping plans.
A: There is no irrigation in the project.
Q11: Are the signs shown on attached drawing A10.1 required on this project?
See below quantities & please confirm.
• Qty 5 Room signs
• Qty 4 Restrooms signs
A: Signs must be ADA compliant with raised letters and braille. The vendor is negotiable; what’s shown on A10.1 are from mydoorsign.com
(4) Room signs:
#113 IT CLOS
#124 CUST
#130 MECHANICAL
#129 ELEC
(4) Restroom signs:
#121 FAMILY RESTROOM
#122 MEN’S BATHROOM
#123 WOMEN’S BATHROOM
#213 H/C UNISEX
Q12: What are the working hours? Are there any work hour restrictions?
A: The library will vacate the building and site for the construction period.
Q13: Could you provide guidelines for the following site logistics?
a) Access for materials delivery and debris removal
b) Access to restroom facilities
c) Requirements for a trailer on-site A: Site logistics are at the discretion of the Contractor. The library will vacate the building and site for the construction period. The Contractor will have full access to the site. The Contractor needs an office space on site; this can be with a trailer or other means.
Q14: Is there a designated fire alarm vendor for this project? If so, please provide the vendor's name.
A: There is no designated fire alarm vendor.
Q15: [proposal for window substitutions] A: Substitutions during the bid period will not be accepted. The windows and doors in the bid package are the Basis of Design. Addendum 2 addressed substitutions with revised sections 000100 Definitions & Standards and 000200 General Information to include the definition for Basis of Design:
“During the Bidding process, these are to be viewed as proprietary and included in the base bids. When additional items are listed in the specifications after a product identified as “Basis of Design”, they may be proposed after contract execution as a change and will be examined by the architect and accepted or rejected.”
Clarifications:
1. Regarding the Hazardous Material abatement in the Reading Room: Wyeth Architects suggests considering encapsulating the areas of concern with appropriate paint per the State’s guidelines included in Addendum #3.
