About this file
This is a Disabled Veteran Business Enterprise (DVBE) Participation Statement form for the Azusa Unified School District, a California K-12 public school district seeking to award a general building contractor bid for a walk-in refrigerator installation at Victor Hodge Elementary School located at 700 W. 11th Street, Azusa, CA 91702. The project is funded in whole or in part by the State of California pursuant to the Leroy F. Greene School Facilities Act of 1998. Bidders are required to complete the DVBE Participation Statement as a mandatory bid form demonstrating reasonable efforts to secure participation by disabled veteran business enterprises as subcontractors and/or material suppliers. Bidders must certify either that they were unable to secure DVBE participation after reasonable efforts or specify the anticipated dollar amount and percentage of DVBE participation in the contract. The form requires the contractor to report actual DVBE participation upon project completion, including amounts in any change orders issued during construction.
The bid package includes mandatory documents such as a bid bond, performance bond, payment bond, proof of insurance, contractor certification regarding background checks, and a project labor agreement requirement. Contractors must provide references from K-12 public school districts and document current and completed modernization projects as prime contractor. Insurance requirements include builder's risk coverage with minimum aggregate limits of $2,000,000 per occurrence and $1,000,000 for pollution liability. The contract emphasizes contractor responsibility for safety, permits, labor compliance, and wage requirements, with provisions for district oversight through a project inspector and architect administration of the contract.
