About this file
This is a Pre-Qualification Questionnaire document issued by Azusa Unified School District in California for contractors seeking to bid on public works projects, including the Walk-In Refrigerator project at Victor Hodge Elementary School located at 700 W. 11th Street, Azusa, CA 91702. The questionnaire requires prospective contractors to provide comprehensive information about their firm's organization, structure, financial standing, licensing status, bonding capacity, insurance coverage, safety record, and historical performance on public and private construction projects. Contractors must submit detailed responses covering six recently completed public works projects and three largest private projects within the last three years, along with current contact information for project owners, architects, engineers, and construction managers. All responses must be certified under penalty of perjury by authorized company representatives, including corporate officers for corporations or general partners for partnerships.
The questionnaire establishes essential criteria that all bidders must satisfy, including possession of valid California contractor or professional licenses, current DIR Public Works Contractor registration, liability insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate, current workers' compensation insurance or legal self-insurance, and reviewed or audited financial statements. Additionally, bidders must provide a notarized statement from an approved surety insurer confirming current bonding capacity. The document contains extensive sections addressing contractor history over the preceding five years, including bankruptcy declarations, tax liens, civil litigation, debarment actions, safety violations, prevailing wage compliance, and apprenticeship law violations. Contractors must disclose any surety bond defaults, insurance policy denials or non-renewals, CalOSHA citations, EPA citations, and any periods without workers' compensation insurance coverage, with detailed explanations required for violations or negative occurrences.
