ATTACHMENT 2: 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
ADDENDUM to 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
(a) Basis for Award: The Government will issue a Firm-Fixed-Price (FFP) Indefinite Delivery, Indefinite Quantity (IDIQ) contract to the responsible offeror whose offer, conforming to the solicitation, represents the lowest price and whose price is determined to be fair and reasonable.
(b) The following factors shall be used to evaluate offers:
1. Price
(c) Special Standards of Responsibility: In addition to the general standards of responsibility under FAR 9.104-1, offerors must meet the following special standards of responsibility in accordance with FAR 9.104-2:
1. Past Experience: The offeror/prime must demonstrate relevant past experience to NAICS 621910 within the last five years either 1) performing non-emergent ambulance transport services under contract themselves or 2) managing provision of non-emergent ambulance transport services under contract as a prime contractor. This shall be demonstrated by submission of the following for at least two, but not more than five, Government or Commercial contracts:
a. Contract, task order, or purchase order number [if applicable].
b. Period of performance dates.
c. Point of contact (name, title, and phone/email).
d. Brief description of the services performed.
2. Licensing/Certification – for at least two (2) current employees:
a. Proof that drivers are properly licensed for the class of vehicle driven.
b. Proof that drivers have a special transportation service vehicle permit, or endorsement issued by the commissioner of public safety (example: California Ambulance Driver Certificate).
c. Proof that drivers have successfully completed the standard and advanced first aid course of the American Red Cross of US Bureaus of Mines or equivalent.
d. Proof that all medical personnel (EMT, CCTP, etc.) providing services on the contract are properly certified and licensed for the tier that they are performing services.
3. Insurance:
a. Proof of Indemnification and Medical liability insurance.
b. Proof of fleet vehicle insurance.
Failure to demonstrate compliance with these special standards will render the offeror non-responsible and ineligible for award.
(d) Evaluation Approach: The Government will first review quotations for compliance with the solicitation requirements, including the required submittals listed on Attachment 1: 52.212-1 Instructions to Offerors Commercial Products and Commercial Services. Quotations deemed compliant will then be evaluated for offeror responsibility in accordance with FAR 9.104, including any applicable special standards of responsibility identified in the solicitation. Among quotations that pass both the compliance and responsibility reviews, price will be the sole factor considered. The evaluation will consider the following:
1. Compliance Review: The Government will review each offer for compliance with the solicitation, including submission requirements. Non-compliant offers will be excluded from further consideration.
2. Responsibility Determination: The Government will determine responsibility in accordance with FAR 9.104-1and FAR 9.104-2.
3. Price: The Government will evaluate the price by adding the total of all line-item prices, including all options, to determine price reasonableness in accordance with FAR 13.106-3(a). The Total Evaluated Price will be that sum.
(e) Options: The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by adding six months of the quoter's price for the last potential period of performance under this contract to the quoter's total price. Quoters must price only the base and option periods specified in the contract line-item numbers (CLINs) and shall not submit a price for the potential six-month extension of services under FAR 52.217-8. The Government reserves the right to exercise the option under FAR 52.217-8 before the end of any of the contractor's performance periods and will be exercised at the price and/or rates in effect at the time the clause is exercised. Evaluation of options shall not obligate the Government to exercise the option(s).
(End of Provision)
