The incident resulted in the death of four workers and the injury of at least one other worker. The explosion severely damaged the production facility and impacted neighboring facilities. The CSB deployed a team to Waukegan, Illinois, on May 3, 2019, to investigate the multi-fatality incident that occurred at AB Specialty Silicones. The CSB and its experts needed to enter the facility where the explosion occurred. The facility had multiple structural and health hazards. An urgent and compelling need for this contract exists in accordance with FAR 6.302-2. Family impacted by this tragedy, which includes the loss of life, are awaiting some results from this work before personal belongings can be returned. The facility has been exposed to the elements, and the longer it is exposed, the more likely evidence will be lost.
Due to the unknown health hazards, the CSB and its contractors urgently need an industrial hygienist who would be willing to enter the structure with time frame allotted by other contractors creating an urgency to find a certified IH who could enter the facility and provide needed air monitoring and characterization of safety hazards.
5. Any other important information. Brief market research was conducted. However, due to urgency, and the nature of the requirement, competitive options would not be appropriate.
Future acquisitions of this kind will be evaluated case by case and competitive options encouraged if possible. A written Notice to Proceed was given to Scopus Consultants by the Contracting Officer on July 3, 2019 citing FAR 6.302-2.
