About this file
This is a Price Increase Percentages form for the Arizona Department of Transportation (ADOT) Solicitation No. BPM007111 for Vehicle Glass Replacement, Repair, Tinting and Accessories. The form requests that offerors identify the maximum price increase percentage they may request at each renewal period should ADOT exercise its renewal options. The contract has an initial twelve-month term with four potential renewal periods. Offerors must complete the form by entering their requested maximum increase percentages for the 1st, 2nd, 3rd, and 4th renewal periods. The solicitation's proposal deadline is October 17, 2025, with a contract term extending up to forty-eight months total if all renewal options are exercised.
The price increase form specifies that each renewal period's increase percentage shall be computed against the original contract price, not cumulatively against prior renewal periods. In the completed form provided, the offeror requested maximum increases of 2% for the 1st renewal period and 1% for each of the 2nd, 3rd, and 4th renewal periods. ADOT explicitly cautions that the state does not automatically grant any requested increase upon renewal; offerors must provide documentation of need at the time of renewal to justify any price increase. If offerors fail to provide increase percentages, prices will remain unchanged from the original contract term.
