About this file

This document is a COVID-19 safety guidelines and procedures document for the Hines VA Hospital facility.

The guidelines establish mandatory health and safety protocols applicable to all individuals entering the Hines hospital campus, affiliated buildings, and Community Based Outpatient Clinics (CBOCs). All persons must wear masks or face coverings at all times while on the facility. Contractors are required to carry their Personal Identity Verification (PIV) cards at all times. Campus access is restricted after 4:30 PM, with only the Emergency Department and Building 200 main lobby remaining open for entry and re-entry after that time; unauthorized entry through other doors will be enforced by VA Police.

Upon arrival, all individuals must proceed through one of four designated COVID-19 screening points: the main lobby of Building 200, the Atrium north of Building 45, the C-Section east entrance to Building 1, or the main lobby of Building 217. At each screening point, individuals undergo daily temperature checks and symptom screening, after which they receive a dated sticker placed on their PIV card confirming screening completion. Personnel may exit through any normal exterior door but must ensure doors close fully behind them. For re-entry during business hours, individuals who were already screened may simply display their dated sticker; after 4:30 PM, re-entry is limited to the Building 200 main lobby or Emergency Department. The document explicitly prohibits propping open doors or allowing unscreened individuals to enter, with violations resulting in immediate removal from the station and additional penalties. A campus map identifying the four screening point locations is provided.
