About this file
This is a submittal package from KCSR Inc. dba Pestmaster Services of Tucson in response to the Arizona Department of Economic Security's Request for Proposal BPM006864 for pest control services at eligible ADES facilities throughout Arizona. Pestmaster proposes comprehensive integrated pest management (IPM) services utilizing non-chemical methods as primary controls, including sanitation, exclusion, caulking, vacuuming, trapping, monitoring, and harborage removal, with pesticide application reserved as a last resort using EPA-exempt and reduced-risk products. The proposal includes monthly service visits with full interior and exterior inspections, application of exterior chemical barriers around buildings, placement of monitoring boards, documentation of findings, and proactive identification of facility maintenance issues. Pestmaster indicates it currently holds an active contract with ADES for statewide facilities since December 2019 and is positioned as the incumbent provider. The proposal was submitted by the March 9, 2025 deadline, with the contract addressing pest control needs across multiple ADES locations statewide. The contract term is not explicitly stated in the submittal package.
Pestmaster's pricing includes monthly service charges with a proposed 10% reduction if awarded as the sole provider, plus separate pricing for emergency call-outs ($250), termites ($9.50 per linear foot), bed bugs ($3.50 per square foot), and rodent bait stations ($12 per station), with various services marked "TBD." Payment terms are NET 30, with pricing held constant during the contract term unless the scope of work changes. All equipment remains Pestmaster property and will be removed upon contract termination. The company demonstrates qualifications through extensive experience in government contracting, holding 16 active contracts with various government agencies and tribal entities, and employing technicians with combined pest control experience exceeding 30 years, all properly licensed in Arizona. Pestmaster maintains commercial general liability insurance with $2,000,000 limits per occurrence and $4,000,000 aggregate, workers' compensation coverage, and pollution liability coverage of $1,000,000. The proposal includes current Arizona pest management applicator certifications for four technicians, all expiring May 31, 2025.

Service centers across Arizona, California, D.C., Nevada, Georgia, Kansas, Maryland, New Jersey, Florida, Pestmaster Services -- Who We Are
Pestmaster Services was founded in April 1979 with a vision of providing the most effective pest and vegetation management services possible, while maintaining an earnest concern for our environment and safety. Our level of customer satisfaction is high. We have received many awards and letters of appreciation from both government agencies and private contractors. We have expanded and currently have more than 40 franchise locations all across the United States.
Pestmaster Services of Tucson is owned by Kat Cartwright and her four franchises cover 80% of Arizona and all of New Mexico. Pestmaster Tucson was founded by Kat Cartwright on November 1, 2015 with one small franchise located in Tucson, AZ. Over the past 9+ years, she has expanded to four franchise territories and grown the business from its startup revenue of approximately $100,000 per year to $1.1 million per year, of which 85% or more is due to Navajo Nation contracts for pest, weed, and bird control. Pestmaster has built a stellar reputation with our clients because they know they can depend on us for long-term results and minimal negative impact to people, wildlife, and the environment.
Integrated Pest Management
Pest prevention begins with two primary factors: (a) the correct identification of the pest and knowledge of its needs and entry points and (b) application of an exterior barrier around each building that prevents pests from entering the buildings. Our Integrated Pest Management approach is a pest management strategy that focuses on long-term prevention of pests through a combination of techniques such as monitoring, habitat modification, improving sanitation, and employing both mechanical and physical controls prior to using any pesticides.
Our company will complete this in “green” fashion, with an emphasis on the newest materials and strategies in pest management. This includes:
Reliance on monitoring and inspection Recommendations on sanitation and exclusion of pests (versus simply
“spraying” bugs) Use of EPA‟s list of “Exempt” and “Reduced Risk” pesticides, which are so low toxic, they are considered “food grade”
During our 40+ years in business, our company has built its reputation on the safe and effective procedures of IPM. That has led to the development of a national pest control franchise program, Pestmaster Franchise Network, which has IPM at the heart of its “Mission Statement”. Education, training, and new procedures are always part of our monthly training seminars.
Pestmaster Services vehicles are always equipped with all necessary safety equipment and documents, including MSDS Sheets, Safety Plans, and Quality Control Plans. All Pestmaster vehicles are clearly marked with our company decals and contact information. Our technicians are also always easily recognizable with their clean, professional uniforms and ID badges.
Pestmaster Services has developed relationships with both suppliers and developers of new technology along with our customers to ensure the continuing transfer of new technological advances throughout our organization. We work closely with suppliers and developers to test products and to train our employees. We also believe in sharing the new technologies with our customers to promote the most technologically sound IPM services available.
Pestmaster Services has gained a tremendous amount of experiences and confidence in several lines of products available on the market which are made from food grade materials, are either low or exempt and have proven to be effective weapons against target pests named in this Bid.
Some of these products, known as “Exempt”, do not even have an EPA number, because they are not considered a true “pesticide”. They work largely on nerve receptors that insects have and that mammals do NOT have and therefore have no

Service centers across Arizona, California, D.C., Nevada, Georgia, Kansas, Maryland, New Jersey, Florida, effect on people or pets. That‟s why, in sites identified in this pest group, the introduction of this technology would be especially well suited for your facility
The above practices result in our technicians being trained by the actual developers and manufacturers of the latest techniques and products prior to their release on the “open” market. We in turn pass this knowledge and product availability on to our customers. The consequence is that our client base has the most advanced and effective IPM service available.
Pestmaster’s Added Value
Dealing with an experienced and professional vendor that currently performs service on multiple government and commercial facilities nationwide will streamline response time and reduce security concerns. Pestmaster Services is dedicated to the development of effective pest management programs that support a healthy living and working environment.
Pestmaster Services is able to offer additional services to your site as well, including Pest Control, Vegetation Management, Bird Control, Termite Treatments, Vector Control and COVID-19 Decontamination. Pestmaster Services is uniquely qualified to bring multiple services to your locations, all through one supplier.
Our technicians are also trained to be a second set of eyes while on your property and will immediately report any suspicious activity witnessed.
Management/Quality Control Approach
The significance and importance of maintaining a pest free environment on your facility requires full control of all variables so that smooth and consistent operations can continue. Every aspect of operations is important, as any small problem can develop into a large problem quickly. With pest management in facilities, what seems like a small problem such as a few ants can quickly become a significant problem when a dining facility is over run by ants and food is ruined.
All employees working on this project will be properly licensed and fully trained in proper pest and rodent control, safety protocols in general, and specific safety protocols related to work at power substations. All employees will be supervised, directly or indirectly, by our VP of Operations, who will also be responsible for training in all matters. In addition to meticulous review of every inspection report submitted by our technicians, Pestmaster will conduct random QC checks to ensure that all employees are achieving our goal of zero-tolerance for pests.
Innovation and Proactive Attitude / Integrated Pest Management
As has been said before, we pride ourselves on the fact that we work towards becoming a proactive partner with our clients. We don‟t want to be just another contractor; we prefer to take more ownership in the services we perform.
In the case of this project, while certain types of work are specified in the scope of work, we will always be considering what else would be in the best interest of the facility. If there is another pest or maintenance issue we feel needs attention, we will review that with the Point of Contact so a course of action can be developed.
Pestmaster Services continuously researches technological innovations in the fields of pest control. We always want to stay on the cutting edge and provide our clients with the most effective methods while maintaining asincere concern for the environment. A few of the items we may suggest at certain times for pest control are the use of simple soaps for control of certain pest species or use of a mint oil, pesticide free product for pest or bee control if the need exists. As other new materials or procedures are introduced, we will bring them to the table – all in the spirit of continually refining and making the IPM Program for * facility the best value now and in the future.
Once the basic principles have been understood, there is no substitute for resourcefulness and ingenuity in developing practical, site-specific pest management solutions. It is incumbent upon all personnel involved in the pest management program to actively support IPM initiatives and provide resources for implementation. This includes trained Pest Controllers, Pest Control Quality Assurance Evaluators, and through our continuing education processes for personnel.
Pestmaster - Page 4
Service centers across Arizona, California, D.C., Nevada, Georgia, Kansas, Maryland, New Jersey, Florida, Pestmaster technicians are trained to determine which methods are best in any given situation. Our priorities will be to use non-chemical approaches. Our specific methods include, but are not limited to: vacuuming, screening, caulking, removing harborage, and other exclusion methods. If these are not practical or possible, we will then consider other options, starting with low toxic alternatives like baits or gels, and ending with a more traditional pesticide application only after all other options are eliminated. Regardless of the IPM method employed, we will monitor the result to ensure that the pest control needs of the client are addressed.
Project Approach
We will work with DES staff to set up a regular servicing schedule convenient to both parties and will send out reminder emails to each location at least one week prior to each service.
Each time we arrive for a scheduled service, our technician will fully inspect both the interior and exterior of each building to determine what pest issues (if any) currently exist and to make notes of any maintenance issues which may need to be handled to remove access, harborage, or other issues which are attracting pests to the facility. Once the inspection is complete, the technician will install monitor boards as appropriate and will apply safe and effective products (if appropriate) to control any and all pest issues which currently exist. In addition, Pestmaster will apply a chemical barrier around each building (weather permitting). It is this exterior barrier which keeps pests outside and prevents them from entering the building. Pestmaster will not apply any products on the interior of any building unless there is current visible evidence of pests. We do not ascribe to the "spray jockey" method of pest control, where chemical products are sprayed all over inside of a building even if no pest problem currently exists. This method of pest control (used, unfortunately, by many companies big and small) is not only ineffective, but is not safe for your staff, your clients, or the environment.
In addition, unlike any other pest control provider, Pestmaster of Tucson has developed its own 100% natural product which has proven to be highly effective in eliminating problems with mice and other burrowing rodents. It is also proving to be a highly effective barrier to keep bugs, spiders, and other pests out of buildings.
While there are different types of pests, the concept of an effective work plan is implementing a true IPM approach. We manage the pest with the most effective yet safest environmental methods. Our strategy is:
Inspect all locations and facilities
Record and define all pest activity Develop a plan/strategy for each pest Establish action thresholds Monitor populations of pests Control the pest (if required) Document the result Regularly evaluate and redesign the program Recommend to COR best methods for limiting pest entry
Inspection
In the diagnosis of any pest presence, the question which must be addressed is „Why are the pests present?‟ The first step of IPM, the inside inspection, usually answers this question. Our entomologist, sanitarian, and operations managers are available to inspect and review the availability of entry points for pest species, sanitation issues, actual current pest infestations, and any other issues conducive to pest infestation. During our inspections, we place pesticide-free monitoring boards in strategic locations. These monitoring boards assist our technicians in determining what pests and which exact species are present, as well as to help us determine which direction the pests are coming from/going to.
When we inspect the interior of any building some of the main areas we will inspect if applicable are the kitchens, cafeterias, break rooms, and vending/coffee area. One of the first things we look at is the floors: are they clean, is there material stacked on them? Are the floor drains functioning and empty? Can you access and clean under cooking equipment and are the grease traps
Pestmaster - Page 5
Service centers across Arizona, California, D.C., Nevada, Georgia, Kansas, Maryland, New Jersey, Florida, clean? Are trash and spills cleaned as they occur? In the storage areas, are the supplies kept on wire shelving, are supplies taken out of cardboard boxes? Is there a locker area? Is there food present in the lockers? Are cleaning supplies up off the floor and stored properly? In the dining areas, are the table legs cleaned? Is the drink and condiment area cleaned at least daily? Are the food trays cleaned and dried before they are put away? Are all cracks and crevices sealed?
The second part of this process will be a full and complete inspection of the perimeter of the building. Pest infestations usually come from outside the building. This will give us an overview of the possible habitats and transportation corridors for the pests.
It will also show where the possible entry points are. By inspecting outside, it helps us to know where to concentrate inside.
The inspection process, including the formation of the pest control plans for the facility, is absolutely the most important step and should not be done is a rushed manner. We will arrange to meet the Point of Contact and devote whatever time is needed to ensure a complete inspection inside and outside of the facility.
Non-chemical pest control methods are implemented or recommended wherever feasible. These methods will include:
Sanitation Exclusion/Caulking Interception Vacuuming Trapping and monitoring Harborage Removal Environmental alteration Lighting
A brief explanation of each of these methods follows:
Sanitation: Sanitation involves cleaning up and removing potential food and water sources for pests. The fewer food sources that exist, the fewer pests that can survive. Also, pests will be more drawn to the baits when there are fewer options around for them to feed. We recommend a regular schedule of cleaning floors, equipment, floor drains, trash receptacles, and dumpsters as the best approach to minimize food sources for pests. We also recommend that water leaks be repaired promptly and wet mops hung up to dry.
Exclusion: Exclusion involves sealing up cracks and holes where pests can enter. This is accomplished by caulking and the use of screens or steel wool. Exclusion is the most effective method to prevent pests from entering a building. We perform minor exclusion as necessary and recommend structural modifications when we observe the necessity.
Harborage Removal: The less shelter or nesting material that is available, the fewer number of pests that will be attracted to an area. We identify and recommend the removal of piles of debris (both indoors and outdoors), eliminating cardboard boxes in storage rooms, and properly eliminating weeds around the exteriors of buildings - all of which are examples of harborage removal. Having fields of weeds near the exterior of buildings provides a natural and safe corridor for pests to enter the building. Pestmaster Services can provide weed control to eliminate this harborage.
Environmental alteration: Changing the environmental conditions of a room or area so pests cannot survive there can be an effective long-term strategy. For example, a wet crawl space under a building can serve as a source of infestation for many pests. Installing ventilation and vapor barriers to dry out the crawl space dissuades most pests from living under the building.
As conducive conditions are observed, we report them to the COR.
Interception: When building occupants or workers examine goods and items for pests as they are brought onto the facility, they can often prevent numerous introductions of pests, including German cockroaches. This is especially important for food items, in particular bagged or boxed produce. We discuss this IPM method with the appropriate staff during regular inspections.
Trapping and monitoring: Rodents can be effectively controlled in many situations using traps alone. For insect control, traps work best for monitoring activity of insects and other arthropod pests. These techniques are used during our regular scheduled inspections where it is feasible to use traps. No pest control material or trap will be placed where it mightbe recovered by any adults, children, or interior pets, nor will such material be allowed to contact food or cooking utensils.
Pestmaster - Page 6
Service centers across Arizona, California, D.C., Nevada, Georgia, Kansas, Maryland, New Jersey, Florida, Vacuuming: Physical removal of pests by vacuuming is now widely accepted. This technique is especially effective for cockroach and spider control. Our staff may remove pests by vacuuming as appropriate.
Lighting: Exterior lights can attract large numbers of insects to buildings where they then enter the structure. These insects also serve as food for spiders, which promotes spider infestations. Exterior lighting should always be changed to sodium vapor lamps where feasible to attract as few insects as possible to a building. During our regular inspections, we will note any recommended lighting alterations and report those back to the COR.
Qualifications and Capabilities
Our technicians currently have a combined total of more than 30 years of pest and weed control experience. In addition, we continually update and add to our training with monthly meetings where we teach new methods and introduce new "green" products. Although we have many pest control contracts in both Arizona and New Mexico, more than 85% of our contracts are in the Navajo Nation and our reputation with the Nation is exceptional.
Because we are not a big conglomerate, we can base our business on being able to customize our services and even our invoices for each customer's specific needs. We realize that the needs of DES facilities are different from those of our other customers and we handle your pest control needs in a way that is safe, effective, and convenient for your facility and staff.
Periodically, we will conduct Quality Control Reviews to ensure that your pest control issues are being properly addressed and that any concerns you have are resolved immediately. Should such a QC Review indicate that we need to adjust our methods or our products for your facility, these changes can be made immediately. Our goal is a "pest free" environment for your staff and clients, while at the same time utilizing products which are safe not only for humans, but also for wildlife and the environment.
All services performed under this contract will be performed by Pestmaster personnel. We will not subcontract any portion of the work to nonPestmaster personnel.
All Pestmaster personnel who may be performing services will be licensed in the states in which they will be servicing and will be fully trained in safety protocols regarding working in power substations.
Pestmaster will provide DES with all required licensing, safety manuals, proof of completion of safety training, and any and all other documentation DES needs within 10 days after award of contract.
Conclusion
We hope the information presented above will provide sufficient detail on our capability to not only meet the Statement of Work requirements, but also show our ability to recognize other potential areas where our pest management services will be in the best interest of DES staff and clients. It is our dedication to detail that has provided our rise in the pest and vegetation control industry over the past 40+ years.
We appreciate having the opportunity to submit this proposal and feel that the diversity of our resume, backed by our commitment to developing a long-range partnership with DES will provide a winning, productive, and cost-effective program not only for the services under this Solicitation, but for future requirements identified.
We look forward to the opportunity to working with you!
Thank You for this Opportunity!

3b If on line 3a you checked “Partnership” or “Trust/estate,” or checked “LLC” and entered “P” as its tax classification, and you are providing this form to a partnership, trust, or estate in which you have an ownership interest, check this box if you have any foreign partners, owners, or beneficiaries. See instructions

Part I Taxpayer Identification Number (TIN)
Enter your TIN in the appropriate box. The TIN provided must match the name given on line 1 to avoid backup withholding. For individuals, this is generally your social security number (SSN). However, for a resident alien, sole proprietor, or disregarded entity, see the instructions for Part I, later. For other entities, it is your employer identification number (EIN). If you do not have a number, see How to get a
TIN, later.
Note: If the account is in more than one name, see the instructions for line 1. See also What Name and
Number To Give the Requester for guidelines on whose number to enter.
Social security number or Employer identification number
Part II Certification
Under penalties of perjury, I certify that:
1. The number shown on this form is my correct taxpayer identification number (or I am waiting for a number to be issued to me); and
2. I am not subject to backup withholding because (a) I am exempt from backup withholding, or (b) I have not been notified by the Internal Revenue
Service (IRS) that I am subject to backup withholding as a result of a failure to report all interest or dividends, or (c) the IRS has notified me that I am no longer subject to backup withholding; and
3. I am a U.S. citizen or other U.S. person (defined below); and
4. The FATCA code(s) entered on this form (if any) indicating that I am exempt from FATCA reporting is correct.
Certification instructions. You must cross out item 2 above if you have been notified by the IRS that you are currently subject to backup withholding because you have failed to report all interest and dividends on your tax return. For real estate transactions, item 2 does not apply. For mortgage interest paid, acquisition or abandonment of secured property, cancellation of debt, contributions to an individual retirement arrangement (IRA), and, generally, payments other than interest and dividends, you are not required to sign the certification, but you must provide your correct TIN. See the instructions for Part II, later.

What’s New Line 3a has been modified to clarify how a disregarded entity completes this line. An LLC that is a disregarded entity should check the appropriate box for the tax classification of its owner. Otherwise, it should check the “LLC” box and enter its appropriate tax classification.
New line 3b has been added to this form. A flow-through entity is required to complete this line to indicate that it has direct or indirect foreign partners, owners, or beneficiaries when it provides the Form W-9 to another flow-through entity in which it has an ownership interest. This change is intended to provide a flow-through entity with information regarding the status of its indirect foreign partners, owners, or beneficiaries, so that it can satisfy any applicable reporting requirements. For example, a partnership that has any indirect foreign partners may be required to complete Schedules K-2 and K-3. See the Partnership Instructions for Schedules K-2 and K-3 (Form 1065).
Purpose of Form An individual or entity (Form W-9 requester) who is required to file an information return with the IRS is giving you this form because they
