About this file
This is a Price Proposal Form for RFP No. ISD_384202_DA issued by Fermilab for the MINOS High Bay Roof Replacement and Masonry Wall Repair project. The proposal was originally issued August 1, 2026, with a revision dated September 25, 2026.
Offerors must provide pricing in two phases: Phase 1 requires a Lump Sum Base Price for three line items including masonry wall investigation only, roof replacement, and a leak detection test to be performed one year after installation (which must be priced separately from the roof replacement). Phase 2 requires all-inclusive unit pricing for masonry and parapet wall repair work across thirteen line items (003-A through 003-M), including tuckpointing, brick replacement, coping stone resetting and replacement, through-wall flashing and membrane installation, weep hole restoration, joint sealing, roof membrane integration, CMU backup grouting, parapet anchors, drainage restoration, and thermal bridging mitigation. Quantities for Phase 2 are estimated for bid evaluation purposes only, with final quantities to be determined after field investigation and field-verified jointly by the subcontractor, architect/engineer, and FFDG; offerors will be paid at their proposed unit prices regardless of final quantity adjustments. All unit prices must be all-inclusive of labor, materials, equipment, scaffolding, disposal, overhead, profit, incidentals, and bonding, with no additional mobilization or equipment fees permitted for quantity adjustments. No masonry unit-price work may proceed without prior written authorization for Phase 2. The proposal acceptance period is ninety (90) days from the opening date unless the offeror specifies a longer period.
