About this file
This document is an FHWA-WFLHD (Federal Highway Administration - Western Federal Lands Highway Division) Equipment Inspection Report form designed to comprehensively document the condition of equipment during an inspection. The form provides detailed guidelines for conducting thorough equipment inspections, requiring inspectors to examine the equipment under good visibility conditions and ensure it is free of mud, dirt, soil, organic debris, sand, gravel, or plant parts.
The inspection checklist covers an extensive range of equipment components, including cracks, crevices, panels, undercarriage, tire treads, bumpers, mechanical systems (transmission, brake mechanisms, axles), exterior and interior elements, and accessories. Inspectors must use tools like flashlights and mirrors, and require contractors to facilitate the inspection by turning on machines, opening panels, spinning tracks/wheels, and providing previous location and equipment details. The form allows for marking each inspected area as clean or not applicable, with a final determination of whether the equipment PASSED or FAILED the inspection.
