Example Format for Past Performance Consent Letter For use with MACC Source Selection
Past performance information concerning subcontractors, teaming partners, and joint ventures partners cannot be disclosed to an Offeror without the subcontractor's, teaming partner's, or joint venture partner’s consent. In an effort to assist the Government's Past Performance review team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the subcontractors, teaming partners, and/or joint venture partners identified in your proposal. The completed consent forms shall be submitted as part of your Past Performance Volume.

TO: CONTRACTOR REFERENCES
SUBJECT: Multiple Award Construction Contract (MACC) Present/Past Performance Assessment
1. The AFNWC/PKOD is in the process of selecting a contractor for award under Multiple Award Construction Contract (MACC). In general terms, this contract will be utilized to acquire a full range construction effort at Kirtland AFB. The length of this contract will be for a base year with four option periods. The Government wants to be confident that the contractor selected will be able to successfully accomplish our requirements.
2. One of the ways the Government intends to evaluate each offer is through the verification of the offers past and present performance on other contracts. For this, we must depend on information received from agencies such as yours, which have had firsthand experience with an Offeror.
3. We have tried to make our questionnaire as quick and simple to complete as possible. General contractor information was provided by the Offeror in Section 1. Please answer the questions that follow in Sections 2, 3, 4, and 5. We would greatly appreciate your comments describing why you rated a particular question the way you did. We ask that you complete and return via mail or email no later than 10 working days from date of receipt. If you have any questions or difficulty completing the questionnaire, please contact:
