About this file
This document is a pre-renovation asbestos survey report for the Community Center Building located at 59 Winburn Way in Ashland, Oregon. The survey was conducted by Coleman Creek Consulting, Inc. on behalf of the City of Ashland. The survey identified the presence of asbestos-containing materials, including 5% chrysotile asbestos in the tan vinyl floor of the side entry landing near the main hall stage (approximately 30 square feet) and in the popcorn ceiling of the foyer and main hall (approximately 1,600 linear feet). These findings will inform the upcoming Community Center and Pioneer Hall Rehabilitation project, which is being procured by Jackson County, Oregon.
The rehabilitation project scope and details are not provided in this document. Interested parties should refer to the separate state and local contract opportunity details for further information on the project, such as the full scope of work, response requirements, award timeline, and contract terms.

PRE-RENOVATION ASBESTOS SURVEY
OF
COMMUNITY CENTER BUILDING
59 WINBURN WAY, ASHLAND, OREGON
FOR
CITY OF ASHLAND
INTRODUCTION
Coleman Creek Consulting, Inc. (CCC) was retained by the City of Ashland (COA) to perform an asbestos survey of the Community Center Building at the above address. The purpose of the survey was to determine the presence of asbestos containing materials, prior to renovation activities.
ASBESTOS SURVEY
David W. Fawcett of CCC visited the Community Center Building on June 1, 2023, and reviewed the building with Robert Hallberger, COA Maintenance. Mr. Fawcett collected representative bulk samples from the interior and exterior of the Community Center Building. See Site Sample Record Sheets (pages 3-4) for a description of the samples and locations. For a visual review of sample locations, see Asbestos Location Diagram in Appendix A. Bulk sample location photographs are attached in Appendix B.
ASBESTOS ANALYSIS
All samples collected were sent overnight delivery to Eurofins EPK. Samples were analyzed for asbestos using Polarized Light Microscopy (PLM) techniques (EPA Method 600/R-93/116).
The following materials were reported with asbestos.
1. Tan vinyl floor (Sample 23-059A.8) in side entry landing near main hall stage. 5% Chrysotile asbestos. Approximately 30 sf.
2. Popcorn ceiling (Samples 23-059A.16, 17, and 18) in foyer and main hall. 5% Chrysotile asbestos. Approximately 1,600 lf.
3. <1% Asbestos drywall/mud (Samples 23-059A.14 and 19) in side entry landing and adjacent elevated storage room (assumed under wood paneling) and stair wall to attic. <1% Chrysotile asbestos. Approximately 900 sf.
The other twenty (20) samples collected were reported none detected for asbestos. See the laboratory analytical results in Appendix C. For a visual review of the asbestos materials identified, see the Asbestos Material Location Diagrams in Appendix D. Other asbestos materials may be concealed behind walls or beneath newer finish materials.

RECOMMENDATIONS
Asbestos materials to be disturbed during renovation activities should be removed by a licensed asbestos abatement contractor prior to disturbance. Due to the nature of the renovation work, and location of asbestos materials identified, COA may decide to have asbestos abatement specifications prepared, and an asbestos consultant available to oversee abatement activities, perform air monitoring and air clearance testing.
If materials not sampled are encountered during renovation activities, and sample(s) should be collected and analyzed for asbestos.

All samples were received in acceptable condition unless noted in the Report Comments portion in the body of the report. The results relate only to the samples as received and tested. The results include an inherent uncertainty of measurement associated with estimating percentages by polarized light microscopy. Measurement uncertainty data for sample results with >1% asbestos concentration can be provided when requested.
Eurofins EPK Built Environment Testing, LLC ("the Company"), a member of the Eurofins Built Environment Testing group of companies, shall have no liability to the client or the client's customer with respect to decisions or recommendations made, actions taken or courses of conduct implemented by either the client or the client's customer as a result of or based upon the Test Results. In no event shall the Company be liable to the client with respect to the Test Results except for the Company's own willful misconduct or gross negligence nor shall the Company be liable for incidental or consequential damages or lost profits or revenues to the fullest extent such liability may be disclaimed by law, even if the Company has been advised of the possibility of such damages, lost profits or lost revenues. The test report shall not be reproduced except in full, without written approval of the laboratory. The report must not be used by the client to claim product certification, approval, or endorsement by any agency of the federal government. The Company reserves the right to dispose of all samples after a period of thirty (30) days, according to all state and federal guidelines, unless otherwise specified.
Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point count analysis to lower the detection limit and to aid in asbestos identification.
