About this file

This is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for Contract CN331 with Humiston & Moore Engineers (H&M), a consulting firm providing engineering services for the Gasparilla Island State Park project. The change order modifies Task Assignment TA#03 through Change Order CO#01, dated January 8, 2019. The document reflects approval from FDEP representatives including Budget Representative Lisa A. Harper, Project Manager Marshall W. Flake, and Contract Manager Jack Brady, as well as consultant representative Brett D. Moore from H&M and Contract Authority Valinda Subic from FDEP.

This change order authorizes post-permitting services for extended beach fill work at Gasparilla Island State Park, specifically Subtask 5: Pre-Construction Documents and Coordination. The services include final assistance to the U.S. Army Corps of Engineers (USACE) in project coordination, expedited permit finalization, support for FEMA funding applications, and pre-construction coordination with County, State, and Federal offices. The task is valued at a fixed price of $8,270.00 with no reimbursable costs, bringing the total task amount including all previous change orders to $51,224.00. The work is scheduled for 180 calendar days, with an original task execution date of October 5, 2016, and an extended end date of June 30, 2019. Payment is contingent upon FDEP approval of deliverables, which consist of updated permit plans incorporating August 2018 USACE survey data and DEP permit modifications, to be provided in PDF format to FDEP Parks Service. The overall contract CN331 is scheduled to terminate July 27, 2021.
