About this file
This is a Task Assignment Change Order Form (Change Order No. 1) issued by the Florida Department of Environmental Protection (FDEP) for Contract CN331, a beach renourishment project located at Honeymoon Island State Park. The consultant is Humiston & Moore Engineers, with Brett Moore, P.E. serving as the consultant representative, and Ralph Perkins as the FDEP Contract Manager. The change order was approved by FDEP personnel including Budget Representative Tamika Bass, Project Manager Terry McCullum Jr., Contract Manager Ralph Perkins, and Contract Authority Bryan Bradner, with all parties executing digital signatures on February 13, 2025.
The change order modifies Task Assignment 17 for post-construction monitoring services by updating the scope and timing of the 48-month post-construction survey to be conducted in August 2024 instead of August 2025 and eliminating the borrow area from the survey scope. This modification results in a net cost reduction of $9,974.00, reducing the total fixed fee from $174,238.00 to $164,264.00. The change removes two subtasks (3a and 3b) originally scheduled for August 2025, crediting $31,070.00 and $30,334.00 respectively, and replaces them with two new subtasks (3c and 3d) for August 2024 monitoring survey ($25,070.00) and monitoring report and analysis ($26,360.00). The new subtasks involve beach profile surveys conducted by Sea Diversified and engineering analysis by Humiston & Moore Engineers, with deliverables including electronic survey data in DEP format and a monitoring report with aerial photographs. The task end date remains December 31, 2025, with no change in the overall task timeline.
