About this file
This is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for the Hugh Taylor Birch State Park Seawall Repair project. The document involves FDEP, the project owner and contracting authority, and Humiston and Moore Engineers, the consulting firm providing services. The change order was executed on November 8, 2017, and signed by all required parties including the FDEP Budget Representative, Project Manager, and Contract Manager, as well as the consultant representative.
The change order extends the Construction Administration task for an additional 120 days to allow continued oversight through the completion of seawall cap replacement work. The original task end date of April 30, 2017, was extended to November 26, 2017, and further extended to March 26, 2018. This is a time-only change order with no monetary adjustment; the task amount remains fixed at $32,785.03 with zero reimbursable costs. The extension was requested to accommodate approximately 900 linear feet of remaining seawall cap work progressing at approximately 100 linear feet per week, coordination with a separate welcome pavilion construction contract, and 30 days for post-construction requirements.

Subject: Hugh Taylor Birch Park - Time Only Extension
Derek, This email and the attached draft Change Order form are provided to request a task assignment extension, for time only, as we discussed on November 7, 2017. The extension is to allow our
Construction Administration task to continue through the construction process and include an additional 30 days; a total of 120 day extension.
Based upon our conversation, approximately 900 linear feet of seawall cap remains to be replaced under the current contract. Approximately 200 feet of the remaining is ready for a concrete pour and will be completed in the next few days. This leaves 700 linear feet. The project has progressed at about 100 linear feet per week. Allowing for upcoming holidays, coordination of the construction schedule for the welcome pavilion (under separate contract) and 30 days post construction to complete post construction requirements, we request 120 days extension.
Thank you for your attention, Matthew Fleming
