About this file

This document is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for a statewide indefinite delivery contract (CN 202). The change order involves Michael Baker International, a consultant firm, as the primary contractor, with Intertek PSI serving as a subcontractor. The project is located at Highlands Hammock State Park in Highlands County, Florida, and focuses on building electrical and plumbing repairs. FDEP Contract Manager Alyssa Skehan and Project Manager Wayne Thomas oversaw the agreement, which was approved and digitally signed by authorized representatives from both FDEP and Michael Baker International in January 2022.

This change order (Amendment #3) adds scope and fees to Subtask 9 for supplemental asbestos and lead-based paint testing on roofing materials for four residential structures (BL036001, BL036004, BL036005, BL036023, and BL036040) at the park, building upon an initial survey completed on eight residences. The amendment specifies that only roofing core materials will be sampled and tested in compliance with EPA NESHAP and Highlands County Code requirements. The fixed-price cost for this change order totals $1,755.44, comprising laboratory analysis ($360.00) and labor costs ($1,395.44), bringing the total Subtask 9 amount from $11,261.18 to $13,016.62. The original task execution date was November 12, 2020, with an end date of June 30, 2022, and the change order maintains this timeline with an estimated 15-calendar-day duration for completion contingent upon park staff scheduling availability for on-site testing.

Amendment #3 Highlands Hammock State Park - Building Electrical and Plumbing Inspections

Highlands Hammock, Highlands, FL

Statewide Contract CN202, Task Assignment 32

Project Objective: This amendment adds scope and fee to Subtask 9: Lead and Asbestos Inspections, Testing, and Reporting. PSI previously performed a limited National Emissions Standard for Hazardous Air

Pollutants (NESHAP) asbestos renovation survey and lead-based paint (LBP) testing on Eight (8) Structures at the above referenced site (Subtask 9). This additional limited survey/testing and report is being performed to supplement the initial report.

Subtask 9: Lead and Asbestos Inspections, Testing, and Reporting The following four (4) residences noted below will be tested as part of this amendment. These 4 locations are where PSI understands new vent through roof piping (VTRs) will be installed.

• BL036001 – New VTR

• BL036004 – New VTR & kitchen sink re-route

• BL036005 – New VTR & kitchen sink re-route

• BL036023 – Kitchen sink re-route

• BL036040 – New VTR

Through coordination with the lead designer (AVCON), PSI understands that only roofing core materials will be impacted and that the roofs drip edges will not be affected or impacted.

The noted locations of where PSI understands where the kitchen sink have been rerouted from through wall to grade to through building flooring have already been inspected and the areas have been covered in the original inspection/PSI report and no sampling will be required within the interior of the four (4) buildings.

All remaining areas of the eight (8) structures previously not surveyed or additional structures within the

Highlands Hammock State Park are not a part of this project.

This limited supplemental survey proposed by PSI is intended to meet the NESHAP and any Highlands

County Code requirements for limited renovation.

Asbestos Scope of Services: (Roofing core materials only) An on-site survey of exposed and accessible building materials will be conducted by a team of U.S.

Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act (AHERA) accredited inspectors. The inspectors will list materials found hat are suspected of containing asbestos and give a detailed description of their location. Any suspect materials found to be significantly damaged as defined by EPA will also be noted.

PSI will review all past reports, specifications, drawings, and other documents provided by the owner that would assist in identifying the suspect materials associated with the eight structures. This may also include interviewing maintenance personnel, State Park operations personnel or owners for their knowledge of the use of asbestos associated with the fishing pier structure.

Limited Samples of building materials expected to be impacted/disturbed during the electrical/plumbing renovations will be collected according to EPA guidelines, which dictate the number and location of samples to be collected. PSI will attempt to sample damaged areas and areas that are least visible. The following is a listing of the materials that may be sampled during the course of a typical survey. For the purposes of this proposal, only limited roofing core materials will be samples in associated with four (4) structures.

Surfacing Thermal System Insulation (TSI) Miscellaneous

Fireproofing Pipe Insulation Ceiling Tiles Acoustical Plaster Boiler/tank Insulation Acoustic Tiles Hardwall Plaster Breeching Insulation Blown-in Insulation

Ductwork Insulation Floor Tile/Mastic

Vinyl Sheeting Gypsum wallboard Concrete slabs

Roofing materials will be sampled as a part of this project from four (4) structures previously listed.

PSI will analyze all building material samples according to current EPA regulations. Per EPA regulations, if PLM results of friable samples with visual estimation indicate levels between “trace” and less than 10% asbestos, the client has the option to quantify the asbestos concentration by point counting or to assume the material to be asbestos. Point counted friable samples with results ≤ 1% asbestos are considered to be non-asbestos per the EPA. Unless otherwise directed by the client, friable samples which are visually estimated to contain less than or equal to one percent (≤ 1%) asbestos will be quantified using the point counting procedure to definitively qualify them as ACM or non-ACM. Friable samples visually estimated to contain >1% to less than 10% asbestos will not be point counted unless requested and authorized by the client. The additional charge for the point count quantification, if needed, is listed as a unit rate in the fees section of this proposal. It is anticipated that up to 24 asbestos samples (each sample collected includes laboratory analysis of up to two layers per sample; additional layers are considered an additional sample) will be collected and analyzed as a part of this project.

The PSI inspectors will develop a quantity of each material identified. The quantity will be listed with the material. Any material that cannot be fully accessed will be noted as not having an accurate quantity due to the inaccessibility of the material. Example: piping that goes from a mechanical room into an enclosed chase. The inspector will report only the quantity observed and note that there is the possibility that additional materials are located in inaccessible areas. Quantities will be given in square footage for surfacing materials, linear feet for piping insulation, and each for mudded joints and other single item materials.

PSI will assess the condition of the suspect materials observed. The material assessment will involve touching the materials to determine whether the material is friable and assessing the material in accordance with EPA guidelines.

Patching and/or restoring of sample locations is not within the scope of work: PSI shall collect representative samples of each homogeneous area of suspect ACM by coring through the material to the base substrate. PSI shall collect samples as required by applicable regulations for the determination of asbestos content. Sample locations shall be randomly chosen to the extent possible; however, PSI shall preferentially collect samples from hidden or obscure locations. Additionally, PSI shall attempt to collect samples from areas of pre-existing damage. With the exception of the roofs, sample core locations shall be encapsulated following sample extraction with a liquid spray encapsulant or by covering with duct tape.

PSI will sample the roofing materials associated with the four (4) roofs scheduled to be impacted in accordance with standard industry protocol. Multiple samples will be collected in order to determine whether the roofing materials contain asbestos. PSI will core through all roofing materials and prepare the sample so that each layer of material is analyzed according to EPA for asbestos content. Cores will penetrate all roofing membranes to the roofing substrate. The owner understands that the coring of the roof will void any warranties that may exist with the roofing system. PSI assumes no liability due to possible roof leaks or damage associated with a roof leak. The core samples taken shall only be used for determining the presence of asbestos within the material. Patching: PSI will make only temporary patches to the roof where samples are collected. The owner is advised to obtain the services of a qualified roofing contractor to make permanent repairs to the roof. PSI will furnish the contractor the location and number of the temporary patches. The owner may want to have the roofing contractor on-site during the sampling in order to patch areas as soon as PSI has completed the necessary sampling.

Limited Lead-Based Paint Testing and Analytical Methodology (only if required at areas of roofing scheduled to be impacted, if any) PSI will provide an experienced and EPA-certified Lead Inspector/Risk Assessor or trained PSI technician in the use of the X-ray Florescence (XRF) to conduct Lead-Based Paint testing. The Scope of the limited work for the Lead-Based Paint Inspection will include the use of an Heuresis, Pb200i, XRF Lead Paint Analyzer to test only the building components to be impacted for the presence of lead-based paints. PSI understands that the client has requested only representative testing of areas to be impacted during renovations and that the HUD protocol is not a requirement for this project.

Inspector/Risk Assessor or trained PSI inspector in the use of the XRF will use an Heuresis, Pb200i XRF Lead Paint Analyzer to determine if lead-based paints are present in association with the electrical/plumbing renovation areas to be impacted of the eight structures. The inspector will check the areas such as walls, ceilings, doors, door jams, windows, windowsills, floors, stairs, piping, ladders, railings, etc. As requested by the client, only measurements of random components will be tested from each interior and exterior painted surface expected to be impacted/disturbed during the electrical/plumbing renovations.

Assumptions:

1. Access to the buildings will be allowed to facilitate needed inspections/survey to be performed together. A FDEP escort is preferred in buildings currently occupied by tenants/residents/park staff.

2. Where access to areas 20 feet above floor level, such as above suspended ceilings or roofs, is required, a ladder or other elevating device will be made available to the PSI inspector.

3. Lighting for the building during the course of the survey.

Deliverables:

• Electronic copy of the final report will be provided in .pdf format. This report will contain the detailed results of materials observed, sampling activities, and material quantification.

• Any hazardous materials found will be identified in the final construction plans (type, location, and approximate quantity).

• PSI will provide staff to attend project pre-bid meeting.

Performance Measure:

Deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.
