Dear Mr. DeLeon, For your review, Michael Baker International, Inc., is attaching a change order proposal summary and man-hour documentation for amending the current task assignment to redo survey, geotech services, and drawings for the revised day use restroom and concession building replacement in lieu of the standard building type provided in the original scope.
Please review the attached Proposal Summary and deliverables and let me know if there are any questions or comments.
Sincerely, Jeff Roberts, PE, RSP1 Associate Vice President / Sr. Project Manager
Fanning Springs State Park – Day Use Restroom and Concession Building
Levy County, FL Statewide Contract CN202, Task Assignment 44, Change Order 1
This Change Order #1 captures the efforts to redo the survey, geotechnical services, and preliminary drawings for the revised day use restroom and concession building from the original task assignment.
Project Objective: Under the original contract, Michael Baker (Consultant) completed a survey, geotech, and 30% plans for the day-use restroom and concession building at Fanning Springs State Park in Levy County, Florida. A standard prototype and sample project was provided by FDEP as a basis of design for the replacement facility. The fee for the original subtasks 1, 2, and 3 were expended.
This change order will address the redesign of the restroom and concession building. The subtasks outlined below will be amended as follows:
Subtask 1 – Survey Services o Addition of Subtask 1.1 Additional survey to capture revised location
Subtask 2 – Geotechnical Exploration o Addition of Subtask 2.1 Additional geotechnical exploration to capture revised location Subtask 3 – 30% Documents o Addition of Subtask 3.1 Revised building design, architectural drawings, MEP drawings, and civil site plan.
Subtask 4 – 90% Documents o Addition of Subtask 4.1 Revised building design, architectural drawings, MEP drawings, structural drawings and civil site plan.
Subtask 1.1: Additional Survey
The Consultant will authorize George F. Young, Inc. (GFY) to perform a Topographic survey on the area of the property outlined above that will include the following:
Topographic survey including visible features and utilities in the area inidcated in Figure 1-A.
Work to be provided in State Plane coordinates and in NAVD 1988.
Figure 1-A. New Requested Survey Area (Shown in Blue)
Assumptions:
1. FDEP will supply access to the site for survey personnel.
2. FDEP will provide plans or other record information for the existing building to be removed and information on any known utilities near the project area or within the park.
Deliverables:
A completed, signed and sealed electronic version of the survey in AutoCAD 2018 “DWG” and PDF trasmitted by electronic means.
Performance Measure: Final deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.

ORIGINAL Subtask 1 TOTAL $6,714.00
NEW Subtask 1 TOTAL (This Change Order) $10,433.02
Subtask 2.1: Geotechnical Services
The Consultant will authorize PSI Intertek (PSI) to perform necessary geotechnical investigations required for building foundation design. The following is the scope of services for this subtask:
Review readily available published geologic and topographic information. This published information will be obtained from the appropriate quadrangle map published by the United States Geological Survey (USGS) and the United States Department of Agriculture (USDA) Soil Conservation Service (SCS).
Execute a program of subsurface sampling and field testing. As requested, the following geotechnical exploration borings will be performed:
o Perform a total of two (2) Standard Penetration Test (SPT) borings to planned termination depths of 25 feet at or near the proposed building footprint.
o Boring locations shall be located in the field using recreational-grade GPS equipment in locations determined to be accessible for performing truck-mounted borings. In the event borings are found to be inaccessible or blocked due to underground utilities, they will be offset to the nearest accessible location.
Collect bulk samples and visually classify and stratify representative soil samples in the laboratory using the Unified Soil Classification System (USCS). Identify soil conditions at each boring location and form an opinion of the overall site soil stratigraphy.
Collect groundwater level measurements in the borings and estimate seasonal high groundwater levels.
Perform laboratory testing results in accordance with the laboratory services listed below.
Prepare a report for proposed site summarizing the information obtained from the field-testing program results and presenting the subsurface soil and groundwater conditions encountered.
Assumptions:
The soil borings can be performed with minimal vegetation removal.
No Maintenance of Traffic (MOT) will be required.
Drilling crew will meet the Park Maintenance Officer onsite to field locate soil borings prior to commencing drilling activities.
Final depths of the borings may be extended or reduced depending on the subsurface materials identified during field activities.
PSI will contact One Call utility locating services for public utility clearance prior to the start of drilling activities. It is our experience that these companies do not mark the locations of privately owned utilities. Private utility lines and other subsurface appurtenances will attempt to be located in the field prior to field activities.
PSI will exercise reasonable caution to avoid damages to underground utilities by contacting local utility companies prior to the field activities. However, private utility locations are often unknown by public utility companies and by the utility owners. Therefore, PSI will not be responsible for damage to the site or any buried utilities if the steps outlined in this scope of services are followed.
Some damage to the ground surface may result from the drilling operations near the work areas. The field crew will attempt to limit such damage, but no restoration other than backfilling the borings is included in this proposal.
In the SPT boring locations, PSI will sample nearly continuously for the upper 10 feet, then at 5-foot intervals thereafter. Groundwater levels will be checked at the time of drilling.
As called for by local regulations, all SPT borings will be filled and sealed upon completion.
Deliverables:
Intertek PSI will provide a geotechnical exploration report in electronic (PDF) version.
Performance Measures: Deliverables will be reviewed by the Department to confirm they are complete, accurate and timely.
