About this file
This document summarizes an Oregon Department of Transportation bridge inspection report. The report details the condition of Bridge 05286 carrying Highway 18 over the Coast Fork Willamette River in Lane County, Oregon. Key findings include spalling and cracking concrete on the deck and girders; pack rust and section loss on steel truss members, stringers, and floor beams; and minor abrasion and cracking of concrete columns, abutments, and pier caps. The bridge's deck, superstructure, and substructure were rated in satisfactory to fair condition. The channel and culverts received higher ratings with minor damage and no culverts present. The report recommends various concrete patching and connector replacement work. Accompanying documents include inspection photos, load rating analysis, and maintenance recommendations.

1120-Efflorescence/Rust Staining There are some areas of hairline transverse soffit cracking with a couple areas of rust staining in the soffit of Span 4.
1130-Cracking (RC and Other) The structural overlay had longitudinal and transverse cracking in the wheel paths prior to the deck seal. The worst area for cracking was towards the east end of the truss span.
1131-Soffit Cracking (RC, PSC) There are some hairline transverse cracks in the soffit.
521-Conc Prot Coating The deck has been sealed.
16-Re Conc Top Flange 1081-Soffit Spalls/Delams/Patches There are several patched areas in the left and right overhangs of Spans 1, 2 and 3.
1090-Exposed Rebar There are numerous small spalls with pieces of exposed rebar in both overhangs of Spans 1, 5, 6 and 7 (PHOTOS A9 & B0).
1120-Efflorescence/Rust Staining There are some hairline transverse cracks with efflorescence in the soffit.
1130-Cracking (RC and Other) There was fairly dense hairline longitudinal and transverse cracking in the wheel paths prior to the deck seal.
1131-Soffit Cracking (RC, PSC) There are some hairline transverse cracks in the soffit.
521-Conc Prot Coating The deck has been sealed.
110-Re Conc Opn Girder/Beam 1080-Delamination/Spall/Patched Area There is a small bottom edge spall in Span 1 - Girder 4 near Bent 1. There is a small spall on the exterior face of Span 3 - Girder 1. There is a small spall on Span 6 - Girder 1.
1130-Cracking (RC and Other) There are shear/flexure cracks up to 0.020" in Spans 1, 2, 3, 5, 6 and 7 at less than three foot spacing. There were large shear cracks in the bearing areas of several of the girders at Bents 4 and 5 that have been repaired.
113-Steel Stringer SEE FATIGUE PRONE DETAIL INSPECTION REPORT FOR ADDITIONAL DETAILS. The radius is small on the stringer cope cuts.
1000-Corrosion The stringers have some areas of top flange corrosion.
1020-Connection There is one missing bolt in the bottom clip angle bracket in the connection for Floor Beam 3 and Stringer 5. The connection at Floor Beam 6 and Stringer 4 has a broken rivet in the left clip angle. There are numerous stringer to floor beam connection where the rivets have been replaced with bolts.
120-Steel Truss
SEE FRACTURE CRITICAL INSPECTION REPORT FOR ADDITIONAL DETAILS.
1000-Corrosion There is minor painted over section loss in the bottom chord at some of the batten plate connections. There is minor freckle and edge corrosion on most of the truss members. There is a small amount of pack rust developing at some of the connections and the batten plates, particularly along the bottom chord.
7000-Damage There is a small ding from a high load impact in the Bent 5 portal. There is a half inch drill hole in the outside channel of the bottom chord between L6L7 left. There is a small ding in the bottom flange of the top chord member U8L9 right approximately ten feet from the deck.
518-Steel Paint The truss was spot painted in 2002. Most of the paint is chalking and discolored. There are some areas where the paint system has failed with active corrosion initiated. This is the typical condition of the paint throughout all of the steel members.
152-Steel Floor Beam
SEE FRACTURE CRITICAL INSPECTION REPORT FOR ADDITIONAL DETAILS.
1000-Corrosion There is minor corrosion along the top flanges. A number of the floor beams have minor painted over pitting in the top and bottom flanges.
1020-Connection There is a missing rivet in the L9 connections at each end of Floor Beam 9.
7000-Damage There is a two inch steel delamination in the bottom flange of Floor Beam 8 under Stringer 3.
162-Stl Gus Plate
SEE GUSSET PLATE INSPECTION REPORT FOR ADDITIONAL DETAILS.
1000-Corrosion Many of the gusset plates have up to an eighth inch deep painted over pitting near the ends of the truss members.
Nearly half of the bottom chord gusset plate connections have active corrosion within the connection. A few of the gusset plate connections have initial pack rust without distortion.
1900-Distortion The inside gusset plate at L8 left has minor distortion caused by construction fit up.
7000-Damage There is a small dent in the bottom of U8 Right.
205-Re Conc Column 1080-Delamination/Spall/Patched Area There are small corner spalls in Bent 2 - Columns 1, 2 & 3, Bent 3 - Columns 1, 2 & 3, Bent 6 - Columns 1 & 2 and Bent 7 - Columns 1, 2 and 3.
1190-Abrasion(PSC/RC) The two columns at Bent 5 have minor abrasion with exposed aggregate.
215-Re Conc Abutment 1120-Efflorescence/Rust Staining There are some hairline vertical cracks with efflorescence in the abutments.
1130-Cracking (RC and Other) There is some hairline vertical cracking in the abutments.
221-RC Spread Ftg 6000-Scour The top of the footings are exposed at Bent 5 (PHOTO B1). Rip-rap was added at Bent 5 around the exposed footings.
234-Re Conc Pier Cap 1080-Delamination/Spall/Patched Area There is a small spall in the Bent 4 cap under Span 3 - Girder 4.
