Remarks
12-Re Concrete Deck Span 2.
1120-Efflorescence/Rust Staining In span 2, some minor cracking w/ efflor in S bay.
1130-Cracking (RC and Other) Dense deck cracking. Worst in EB lanes. Sealed 7/09. Still sealed 2016.
1181-Wheeltrack Rutting Minor rutting (1/8").
39-Prestressed Slab Spans 1 & 3 - P/S slabs.
1080-Delamination/Spall/Patched Area Minor spall in #9, span 3.
511-AC Wearing Surface Water leaking between slabs spans 1 & 3
109-Pre Opn Conc Girder/Beam Span 2.
210-Re Conc Pier Wall Flow of channel against Bent 2 apron. Monitor bank erosion upstream. Island in middle (200' upstream) pushes flow against the south bank. Active erosion.
1130-Cracking (RC and Other) Vertical Non Live Load cracks in the pier wall, measuring up to 0.010" 5/02, same 5/08. One change 5/10. Bent 3 -shear cracking with efflor and rust at middle of wall.
215-Re Conc Abutment 1080-Delamination/Spall/Patched Area Minor spall at Bent 4.
306-Other Joint Bents 1-4 and ends of impact panels. All joints resealed with crafco 5/15. Some leakage 2016.
310-Elastomeric Bearing 2240-Loss of Bearing Area 2015- Bent 1, slab #18 - the elastomeric pad slipped out. Bt 1,Slab #9 slipping and several pads are askew. At Bent 4 only 1 pad is askew. Under a loaded semi you can see deflection on the pads under the wheel path. There is approx 1/2" +/- movement at Bent 4 (visible from the mastic in the railing). Also there is heavy debris impaction behind the bearings from previously failed joints. Bent 4 would get the most road grime since all water on the bridge deck drains to this side. Also the slabs are sitting at an angle to the flat bearing pads. In many locations the filler mastic between the bearings is gone. In summary - my guess is debris impaction is pushing these bearings out as the slabs expand and contract. Recommendation to pressure washing to get the dirt out, resetting the missing pad and the pads that are beginning to squirt out, and setting a simple anchor in front of the problem pads to prevent them from coming out. This could be grout or mechanical. Bridge crew reset bearings 2016.
321-Re Conc Approach Slab Slight impact loading each end of bridge in the E.B. lanes.
980-Approach Roadway Embank Drains functioning on SW corners.
990-Miscellaneous Elements Ped/bike rail. Concrete slope lining for scour protection. Decorative panels on exterior, spans 1 & 3. 5/18-The decorative concrete attachments (4 total) are breaking free of the bridge. Removed by bridge crew.
