About this file

This is a Rates Attachment document from the Behavioral Health Services Department (BHSD) outlining supplemental residential care facility (RCF) services for individuals diagnosed with Serious Mental Illness (SMI). The document establishes service levels and corresponding daily rates for clients transitioning from higher levels of care to community-based residential settings. Services are designed for clients whose symptoms, level of functioning, and psychiatric history necessitate specialized interventions beyond standard RCF offerings. The attachment specifies eight distinct service categories, including an unsponsored patch rate and five tiered service levels, each with defined client populations, eligibility criteria, and required service components ranging from basic assistance with daily living activities to specialized nursing care and end-of-life services.

Pricing for services ranges from $43.56 per day for unsponsored clients up to $150.00 per day for clients with dual diagnoses of SMI and traumatic brain injury. Level 1 basic services are priced at $55.00 daily and include assistance with grooming, hygiene, medication monitoring, and transportation to appointments. Specialized levels include Level 2 at $68.00 daily for monolingual or sensory-impaired clients, Level 3 options at $80.00 daily for clients with behavioral or medical complexities, Level 4 intermediate services at $140.60 daily requiring specialized nursing care, and Level 5 at $150.00 daily for clients with traumatic brain injury. The majority of clients are expected to have Social Security Insurance (SSI) covering basic board and care costs, with BHSD paying only the individualized treatment rate. For unsponsored clients without SSI benefits, BHSD covers both board and care costs and basic facility rate. Should SSI be approved retroactively, offerors must reimburse the County for the daily unsponsored facility rate back to the date of retroactive eligibility approval.

Rates for Supplemental Residential Care Facility Services

The Behavioral Health Services Department (BHSD) is prepared to offer the following supplemental rates for Offeror(s) to provide individualized services for clients who are diagnosed with Serious Mental Illness (SMI) and have a level of functioning, symptoms, and psychiatric history that necessitates supplemental service interventions. Clients are to be transitioned from a higher level of care to the safest and most appropriate residential type of setting based on their ability to function independently.

A. Specialized Individual Treatment Rates

1. For clients who are diagnosed with SMI and need supplemental services, BHSD will negotiate a mental health individualized treatment rate for placement depending on the individual client’s needs. The majority of these clients have Social Security Insurance (SSI) that pays for basic board and care costs. BHSD only pays for the individualized treatment rate which ranges from $55.00 to $150.00 per day depending on the severity of the medical and mental illness. If a client is unsponsored, then BHSD will also pay for the basic board and care costs.

Offeror shall provide residential care facility (RCF) services to clients with SMI whose level of functioning, symptoms, and psychiatric history necessitates service intervention to maintain clients in community settings, and who are not SSI eligible and do not receive benefits. Standard of services will meet Community Care Licensing (CCL) requirements. This includes clients who may suffer from an institutionalized syndrome including increased passivity, lack of confidence, and lack of knowledge about the community.
