About this file
This is Addendum #5 to an Electronic Request for Statement of Qualifications (RFSQ-BHS-FY23-0225) issued by the County of Santa Clara's Behavioral Health Services Department on January 25, 2024. The County seeks qualified firms to provide a variety of services supporting the Behavioral Health Treatment Facility Continuum of Care, expanding the program offerings from seven to ten total programs across four categories. The addendum introduces Category 4, Supplemental Residential Care Facilities, which includes three subcategories: Basic Supplemental Residential Care Services (A4a), Intermediate Supplemental Residential Care Services (A4b), and Residential Care Services for Clients with Severe Mental Illness and Traumatic Brain Injury (A4c). Offerors may apply for one or both levels of care within each category. The addendum modifies evaluation timelines, noting that new providers and existing Collaborative Care Providers (CCPs) applying for new programs follow the Calendar Year 2023-2024 evaluation dates, while CCPs seeking auto-qualification of existing programs may submit documentation on a rolling basis. Contract types for awarded providers may include Short-Doyle/Mental Health Services Act agreements, Acute Psychiatric Hospital contracts, Mental Health Rehabilitation Center agreements, Skilled Nursing Facility agreements, SUTS Master Agreements, or Residential Care Facility agreements.
The addendum updates submission requirements and forms, replacing the COVID-19 vaccine contractor certification with Attachment E (Rates for Supplemental Residential Care Facility Services) and establishing Appendix K as the Proposed Rates Form for Category 4 submissions. Cost proposals for programs A1 and A2 utilize Appendix I, programs A3 utilize Appendix J (Budget Narrative), and programs A4 utilize Appendix K (Proposed Rates Form). Responses must include completed Evaluation Criteria and Qualifications forms (Appendix B1-B4), with Section 2 limited to five pages. The addendum adds new attachments including Sample Residential Care Facility Agreement (Attachment C6) and Sample Critical Incident Sentinel Event Report Form (Attachment F). COVID-19 vaccination requirements have been revised across all existing sample agreements, and declaration submissions include Non-Collusion Declaration, Declaration of Compliance with Equal Opportunity/Nondiscrimination, Wage Theft Prevention, and Declaration of Intent to Comply with County COVID-19 Requirements. All other specifications, terms, and conditions remain unchanged from the original RFSQ.
