About this file
This is an evaluation criteria and qualifications appendix for a state and local contract opportunity issued by a county behavioral health services department for Category 4: Supplemental Residential Care Facilities. The document outlines the scoring framework and requirements for organizations seeking to provide basic supplemental residential care services to adults and older adults with severe mental illness, co-morbid medical conditions, and other specialized needs. The evaluation is structured across five sections with a total of 100 points: Section 1 addresses organizational structure and experience (70 points), Section 2 covers proposed program description (15 points), Section 3 involves historical performance analysis (5 points), Section 4 assesses electronic health record capacity (5 points), and Section 5 evaluates financial solvency (5 points). Offerors must demonstrate experience serving target populations, possess active facility leases or ownership, maintain appropriate licensing and certifications, provide 24/7 services with rapid referral response within 48 hours, and implement trauma-informed, evidence-based practices.
Organizations must document their capacity to meet Medi-Cal requirements, maintain comprehensive staffing plans addressing retention and diversity, provide culturally and linguistically appropriate services across county threshold languages, and establish collaborative partnerships with county departments and other service providers. Applicants are required to submit detailed information regarding personnel retention protocols, emergency staffing plans, pandemic and disaster response plans, outcome evaluation data collection processes, and clinical supervision structures. The evaluation emphasizes organizational commitment to equity, diversity, and inclusion through leadership models and bias mitigation strategies, as well as the organization's ability to facilitate client employment, income maintenance, health insurance enrollment, and permanent housing placement upon program exit.
