About this file

This document is an evaluation criteria and qualifications appendix for a skilled nursing facility category under a behavioral health treatment facility continuum of care request for services (RFSQ-BHS-FY23-0225) issued by a county behavioral health services department.

The RFSQ seeks qualified offerors to provide sub-acute treatment services through skilled nursing facilities serving adult populations (ages 18-59), older adults (ages 60+), and specialized therapeutic services populations, including those with severe mental illness, co-occurring conditions, criminal justice involvement, and diverse ethnic and cultural backgrounds. Evaluation is structured across five sections totaling 100 points: Organizational Structure and Experience (70 points), which assesses the offeror's experience with target populations, Medi-Cal requirements, facility ownership or lease, current licenses and certifications, outcome evaluation data capacity, medication management techniques, staffing composition, staffing-related protocols, timely service implementation capabilities, and pandemic/emergency response plans; Proposed Program Description (15 points), evaluating the organization's vision for equity and diversity, cultural competency and outreach capacity, and collaboration methods with county departments, health systems, and criminal justice partners; Historical Performance Analysis (5 points); Electronic Health Record Capacity (5 points); and Financial Solvency (5 points). Offerors must demonstrate capacity to provide 24/7 services with structured withdrawal monitoring, evidence-based practices, trauma-informed approaches, community residential treatment, timely referral response within four hours, and appropriate step-down services, while maintaining comprehensive data collection and culturally responsive service delivery across all county threshold languages.
