About this file
This document is an evaluation criteria and qualifications appendix for a Request for Service Quotation (RFSQ) issued by a county behavioral health services department for Category 4: Supplemental Residential Care Facilities. The solicitation seeks qualified providers to deliver residential care services to adults and older adults (ages 18 and above) diagnosed with severe mental illness (SMI) and traumatic brain injury (TBI), including those with severe functional impairments, psychiatric and co-morbid medical conditions, and developmental delays. Offerors must demonstrate organizational structure and experience through five evaluation sections totaling 100 points: Section 1 addresses organizational structure and experience (70 points), Section 2 evaluates the proposed program description (15 points), Section 3 analyzes historical performance (5 points), Section 4 assesses electronic health record capacity (5 points), and Section 5 reviews financial solvency (5 points). The evaluation criteria require offerors to confirm experience serving the target populations, possess active facility leases or ownership, maintain appropriate licenses and certifications including Community Care Licensing, demonstrate capacity to meet Medi-Cal requirements, and provide evidence of outcome evaluation data collection processes.
Offerors must document comprehensive staffing protocols including personnel retention plans, staffing plans with organizational charts and staff qualifications, and emergency staffing procedures with quarterly reporting requirements. Additional qualifications include capacity to provide 24/7 services with one-on-one care capability, evidence-based and trauma-informed treatment approaches, intake and assessment processes across all hours of operation, ability to respond to referrals within 48 hours, discharge planning procedures, and step-down service transitions. Organizations must possess pandemic and disaster response plans, cultural competency and linguistic capacity aligned with county threshold languages, and demonstrated collaborations with county departments, contracted providers, and criminal justice system partners. Financial records, electronic health record systems, and historical performance data will be reviewed by subject matter experts within their respective domains using criteria specified in the RFSQ.
