About this file
This is a Proposed Rates Form (Appendix K) from the County of Santa Clara's Behavioral Health Services Department for RFSQ-BHS-FY23-0225, a Request for Statement of Qualifications for Behavioral Health Treatment Facility Continuum of Care services. The document requests that offerors complete applicable sections based on their proposals for three scopes of work: Scope A4a (Basic Supplemental Residential Care Services), Scope A4b (Intermediate Supplemental Residential Care Services), and Scope A4c (Residential Care Services for Clients with Severe Mental Illness and Traumatic Brain Injury). The form does not specify response dates, due dates, site visits, bidder meetings, or award dates; it functions solely as a pricing submission template.
The form requires offerors to propose daily rates per bed for five care levels. Level 1 addresses clients with behavioral challenges including vulnerability, destructive behavior, and assaultive conduct. Level 2 serves monolingual, blind, deaf, or dual-diagnosis clients. Level 3 encompasses clients with serious mental illness, intellectual disabilities, medical frailty, or insulin-dependency needs. Level 4 provides intermediate services for clients with psychiatric and significant co-morbid medical conditions, AWOL history, specialized equipment needs, or end-of-life care requirements. Level 5 addresses clients with severe mental illness and traumatic brain injury with cognitive and judgmental impairments. All proposed rates must include staffing, operating expenses, and associated service delivery costs, with rates subject to mutual agreement between the County and the offeror. Travel costs, if authorized by the County, must be included in the proposed rates rather than reimbursed separately.
