About this file

This is a Past Performance Questionnaire template used by the Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 7 (NCO 7) to evaluate contractor performance on past projects. The questionnaire is submitted by contractors as part of their proposal evaluation for the VISN 7 Construction Multiple Award Task Order Contract (MATOC), an Indefinite-Delivery, Indefinite-Quantity contract for construction services across eight VISN 7 major medical facilities.

The questionnaire requests references to complete a detailed assessment of contractor performance across 26 evaluation criteria using a six-point scale: Exceptional (E), Good (G), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Neutral (N). Evaluation areas include contractor management capabilities, quality control procedures, schedule adherence, personnel management, safety compliance, and performance on specific work elements such as standing seam metal roof installation, brick veneer installation, HVAC equipment and ductwork installation, Direct Digital Controls installation, and Testing, Adjusting, and Balancing of HVAC systems. References must provide narrative explanations for any Marginal or Unsatisfactory ratings. Completed questionnaires must be submitted via email to elissa.goodman@va.gov and joyce.powers1@va.gov, with contact information provided for the Contract Specialist and Contracting Officer. Per FAR 15.201(d)(4), respondent names will remain confidential to the offeror, though responses may be released to the offeror.
