About this file
This is a template form for documenting relevant past contract project experience. The form requires offerors to list up to 15 projects in chronological order by completion date, with the most recent projects listed first. For each project, offerors must provide the following information: project title, contract number, contract value, contracting officer or point of contact name with phone number and email address, a brief description of work performed by the offeror, name of the prime contractor, project location, type of contract (such as IDIQ, single project, or Design-Build), whether pricing was based on RS Means, start date, completion date, and the percentage of work performed by the offeror.
This template serves as a standard form for demonstrating an offeror's relevant experience and qualifications for federal contracting opportunities. The chronological ordering with most recent projects first allows evaluators to assess the offeror's recent and current capabilities. The requirement to identify the percentage of work performed by the offeror and the prime contractor relationship provides transparency regarding the offeror's actual role and responsibility on each project. The inclusion of contract values, types, and pricing methodologies enables federal agencies to evaluate the offeror's experience with similar contract structures and project scopes.
