About this file
This is a Contract Discrepancy Report (CDR) template dated August 2026 used to document and resolve performance or compliance issues between federal contracting officers and contractors.
The CDR process consists of three sequential phases. The initiation phase requires the Contracting Officer's Representative (COR) to complete fields 1-7, which include the contract number, CDR identification number, contractor and COR contact information, a detailed description of the discrepancy or problem (with reference to the Performance Work Statement, Performance-Based Navigation/Navigation Data Service, functional category, and QASP rating of "At-Risk" or "Deficient"), the CDR initiation date, and the contractor response due date set by the COR. The contractor then responds in fields 8-10 by explaining the cause of the discrepancy, proposing corrective actions, and outlining preventive measures to avoid recurrence, along with the date of response and contractor representative contact information. In the review phase, the COR completes fields 11-13, providing an evaluation of the contractor's response (acceptable, partial acceptance, or rejection), a recommendation to the Contracting Officer regarding government actions such as payment withholding, deduction, cure notice, or show cause, and the date the CDR is submitted to the Contracting Officer. The final closeout phase requires the Contracting Officer to complete fields 14-15, documenting the decision on the CDR and whether any contractual actions were taken against the contractor, and signing the document. The CO also communicates government actions via contract modification, provides a copy of the final CDR to the COR, and inputs information into the Accountability Tracker on OPTIC, with reference to ICAM Appendix V for additional guidance.
