General Comments:
Tower Engineering Professionals (TEP) completed a periodic inspection for the above referenced site. The onsite investigation was performed by Tyler Hazen and William Lynch of TEP during the November 12, 2019 and November 13,2019 site visits. The inspection was in accordance with the Scope of Services for tower inspections, maintenance, and repairs of U.S. Coast Guard Towers dated January 2019, the U.S. Coast Guard Tower Manual CIM M11000.4A, FAA AC 70/7460-1L, TIA-322, ANSI/ASSE A10.48, and ANSI/TIA-222-G-2005, Annex J: Maintenance and Condition Assessment (Normative), including all addendums (addendums TIA-222-G-1 2007 and TIA 222-G-2 2009); the checklist is pages 4 thru 9 of this report.
Observations and recommendations are listed herein. For this inspection, the tower legs were named alphabetically according to the position of the ladder per the U.S. Coast Guard Tower Manual CIM M11000.4A.
The overall condition of the tower is poor with a few exceptions. This is detailed in the report.
Chronological Narrative:
TEP arrived onsite at 10:00 am. We began by conducting a tail gate safety meeting with all involved and completing a job safety analysis form. Work began at this tower at 10:15 am with a ground and tower base inspection. All maintenance issues were noted and photographed. TEP left the site at 12:00 pm.
Maintenance Accomplished:
- Minor corrosion was cleaned with hand tools and coated with an approved cold galvanizing compound.
Note:
- No unutilized equipment was observed on the structure.
- No tower identification tag was present on site.
- No USCG assets are present within 150’ radius of the tower.
- TEP did not perform a climbing inspection on this site due to uncertainty that the existing structure allows for a climbing inspection to be performed safely.

EXECUTIVE SUMMARY
Photograph Observations and Recommendations
A.1. Damaged members (legs and bracing)
Observation:
Wooden poles P1 (Photo “A”), P2 (Photo “B”), and P3 (Photo “C”) are severely deformed.
Recommendation:
A licensed structural engineer should analyze the current condition of the wooden support poles.
A
C
B
A.4. Climbing facilities, platforms, catwalks – all secure
Observation:
The existing tower is not climbable and the current condition of the wooden support poles appears unsafe to climb.
(Photos “D”, “E”, and “F”)
Recommendation:
A licensed structural engineer should analyze the current condition of the wooden support poles to determine if they are safe to climb.
D
F
E
B.2. Rust and/or corrosion condition including mounts and accessories
Observation:
Surface corrosion was observed on hardware at the base of the tower. TEP cleaned and galvanized the areas affected by surface corrosion. (Photos “G”, “H”, and “I”)
Recommendation:
No action required.
I
G
H
Observation:
Surface corrosion was observed on all guy wires from the guy anchors to the wooden support poles P1 (Photo “J”), P2 (Photo “K”), P3 (Photo “L”).
Monitor during the next scheduled inspection.

K
Observation:
The guy wires of P2 anchor 2, P2 anchor 3, P3 anchor 1, and P3 anchor 2 are in contact with trees. (Photos “M” and “N”)
Recommendation:
Remove the vegetation in contact with the guy wires.
H.2.d.iv. Guy Hardware Conditions (Cable connectors (Preformed wraps – properly applied, fully wrapped, & sleeve in place))
Observation:
Ice clips are not installed on any of the guy wire preforms on of guy wires on wooden support poles P1 (Photo “O”), P2 (Photo “P”), P3 (Photo “Q”).
Install ice clips.

H.2.d.iv. Guy Hardware Conditions (Cable connectors (Preformed wraps – properly applied, fully wrapped, & sleeve in place)) (cont.)
H.3. Guy tensions
Observation:
All but (2) of the measured guy wire tensions are outside of the recommended range of tensions. Due to questions regarding the stability of the wooden support poles in their current condition, TEP did not adjust guy wire tensions. See Table 1 for details regarding guy wire tensions. See Table 1 for additional guy wire tension information. (Photo “R”)
Recommendation:
A licensed structural engineer should review the current condition of the structure. Properly tension guy wires pending the results of the structural analysis.
