INSTRUCTIONS TO CONTRACTORS
1. The term “material” is defined as articles, supplies, raw materials, equipment, parts, components, and end items that are to be incorporated into the work required by the contract.
2. This form is to be used by contractors for submitting Shop Drawings, Equipment Data, Manufacturer’s Literature and Certificates and Samples of Materials to the Government for approval in accordance with the provisions of this contract. Unless otherwise specified, it is to be prepared in 3 copies, signed, and provided to the Construction Manager with appropriate attachments.
3. Item(s) to be approved will be clearly tabbed or identified. Data pertaining to item(s) to be approved will be clearly identified or tabbed, particularly where documents are voluminous, in order to properly evaluate the materials or articles to be incorporated in the work. Each attachment will be numbered to correspond with the item number shown on the face of this form and the corresponding Submittal Register number contained in the specifications.
4. Requests submitted shall be numbered consecutively, by contract, in the space entitled “Submission No.” This number, in addition to the Contract No., will be used to identify each Material Approval Submittal. Resubmissions will be indicated in the appropriate block and the insertion of previous submission number and date in addition to a new submission number. A single submission should be used for all work of a section of the specifications, but in NO instance should the submission include work for more than one (1) contract. Submittals requiring priority handling will be submitted by separate submittal using the form and so marked across the face of the form.

General Comments:
Tower Engineering Professionals (TEP) completed a periodic inspection for the above referenced site. The onsite investigation was performed by Kelsey Spencer, CWI and Robert Bradley of TEP during the October 9, 2019 site visit. The inspection was in accordance with the Scope of Services for tower inspections, maintenance, and repairs of U.S. Coast Guard Towers dated January 2019, the U.S. Coast Guard Tower Manual CIM M11000.4A, FAA AC 70/7460-1L, TIA-322, ANSI/ASSE A10.48, and ANSI/TIA-222-G-2005, Annex J: Maintenance and Condition Assessment (Normative), including all addendums (addendums TIA-222-G-1 2007 and TIA 222-G-2 2009); the checklist is pages 4 thru 9 of this report.
Observations and recommendations are listed herein. For this inspection, the tower legs were named alphabetically according to the position of the ladder per the U.S. Coast Guard Tower Manual CIM M11000.4A.
The overall condition of the tower is satisfactory with a few exceptions. This is detailed in the report.
Chronological Narrative:
TEP arrived onsite at 8:00 am. We began by conducting a tail gate safety meeting with all involved and completing a job safety analysis form. Work began at this tower at 9:15 am with a ground and tower base inspection. We began climbing the tower at 9:30 am. The climbing inspection ended at approximately 11:00 am. All aerial maintenance issues were noted and photographed. TEP left the site at 12:45pm.
Maintenance Accomplished:
- Minor corrosion was cleaned with hand tools and coated with an approved cold galvanizing compound.
Note:
- Due to accessibility, TEP could only inspect the (6) wooden support poles from the ground.
- No assets were present within a 150’ radius of the tower.
- No unutilized equipment was observed on the structure.
- No tower identification tag was present on site.

EXECUTIVE SUMMARY
Photograph Observations and Recommendations
A.1. Damaged members (legs and bracing)
Observation:
Splitting was observed in all (6) of the wooden poles ranging from 1/2" to 1” wide. (Photos “A” and “B”)
Recommendation:
Have an engineer analyze the towers to determine if repair or modification is required.
B.1. Paint and/or galvanizing condition
Observation:
Cold galvanizing on the diagonal members of the tower is flaking from elevation 40’ to the top of the tower. TEP cleaned and galvanized these members. (Photos “C” and “D”)
Recommendation:
Monitor the tower for corrosion during the next inspection cycle. If section loss or pitting is observed on any tower leg, members, or hardware, consult with a licensed structural engineer to determine if members need to be repaired or replaced.
C
A
B
B.1. Paint and/or galvanizing condition (cont.)
B.2. Rust and/or corrosion condition including mounts and accessories
Observation:
Surface corrosion was observed on the diagonal member bolt connections, the base of the tower, the anchor rod of outside anchor 3, the gussets of the guy connections of outer guy wires 10, 11, 12 at the top of the tower, and the wire rope clips on the safety wire of all guy anchors. Surface corrosion was also noted on anchor rod locations; A-Inner, Outer 3, Outer 5, and Outer 7. TEP cleaned and galvanized the accessible areas where surface corrosion was observed per TIA standards.
Corrosion was observed on the guy hardware at the top of all wooden support poles from the ground. TEP could not determine the extent of corrosion as the poles are not equipped with climbing facilities. (Photos “E”, “F”, “G”, and “H”)
Recommendation:
Thoroughly clean areas of corrosion on the guy hardware at the top of the wooden support poles and coat with at least two coats of ZRC cold galvanizing compound or approved equivalent. During or before cleaning, if any hardware exhibits section loss, pitting, or holes, contact TEP immediately.

F
E
B.2. Rust and/or corrosion condition including mounts and accessories (cont.)
D.1. Connections
Broken ground leads were observed on the ground plate at the base of the tower. (Photo “I”)
Recommendation:
Replace with new ground leads.
I
G
H
H.3. Guy tensions
Observation:
The tensions of outer guy wires 3 and 4 could not be recorded and could not be measured on wires 3 and 4 due to a lack of clearance between preforms to use the Dillon Quick Check.
(Photo “J”)
Recommendation:
Tensions should be recorded with an inline Dillon. If recorded tensions are out of range, re-tension guy wires accordingly.
H.3. Guy tensions
The tension of outer guy wire 8 is above the recommended range of values for tension. TEP did not adjust tensions to avoid excessive increase or decrease in tensions of the wires that could not be recorded. (Photo “K”)
Recommendation:
Tensions should be recorded with an inline Dillon. If recorded tensions are out of range, re-tension guy wires accordingly.
I.1.c. Ground condition (Site condition (standing water, drainage, trees, etc.))
Observation:
Vegetation within the tower compound is overgrown. (Photo “L”)
Recommendation:
Compound should be cleared of vegetative growth and sprayed to prevent future growth.

TOWER TENSION, PLUMB AND TWIST MEASUREMENTS
Table 1: Guy Wire Tensions
* Guy tensions could not be recorded due to lack of exposed area on guy wire between preforms. TEP did not adjust tensions to avoid excessive increase or decrease in tensions of the wires that could not be recorded. This resulted in the tension of (1) wire being outside of the recommended values of tension.
