2. This form is to be used by contractors for submitting Shop Drawings, Equipment Data, Manufacturer’s Literature and Certificates and Samples of Materials to the Government for approval in accordance with the provisions of this contract. Unless otherwise specified, it is to be prepared in 3 copies, signed, and provided to the Construction Manager with appropriate attachments.
3. Item(s) to be approved will be clearly tabbed or identified. Data pertaining to item(s) to be approved will be clearly identified or tabbed, particularly where documents are voluminous, in order to properly evaluate the materials or articles to be incorporated in the work. Each attachment will be numbered to correspond with the item number shown on the face of this form and the corresponding Submittal Register number contained in the specifications.
4. Requests submitted shall be numbered consecutively, by contract, in the space entitled “Submission No.” This number, in addition to the Contract No., will be used to identify each Material Approval Submittal. Resubmissions will be indicated in the appropriate block and the insertion of previous submission number and date in addition to a new submission number. A single submission should be used for all work of a section of the specifications, but in NO instance should the submission include work for more than one (1) contract. Submittals requiring priority handling will be submitted by separate submittal using the form and so marked across the face of the form.
5. This Material Approval Submittal is not valid unless the Construction Manager signs it. This approval is required as called for under the terms of this contract.

General Comments:
Tower Engineering Professionals (TEP) completed a periodic inspection for the above referenced site. The onsite investigation was performed by Chason D. Kanow, P.E., C.W.I. and Kelsey Spencer, C.W.I. of TEP during the October 3, 2019 site visit. The inspection was in accordance with the Scope of Services for tower inspections, maintenance, and repairs of U.S. Coast Guard Towers dated January 2019, the U.S. Coast Guard Tower Manual CIM M11000.4A, FAA AC 70/7460-1L, TIA-322, ANSI/ASSE A10.48, and ANSI/TIA-222-G-2005, Annex J: Maintenance and Condition Assessment (Normative), including all addendums (addendums TIA-222-G-1 2007 and TIA 222-G-2 2009); the checklist is pages 4 thru 9 of this report. No United States Coast Guard Assets were within 150’-0” of the structure.
Observations and recommendations are listed herein. For this inspection, the tower legs were named alphabetically according to the position of the ladder per the U.S. Coast Guard Tower Manual CIM M11000.4A.
The overall condition of the tower is satisfactory with a few exceptions. This is detailed in the report. No unused equipment was identified on the structure.
Chronological Narrative:
TEP arrived onsite at 9:30 am. We began by conducting a tail gate safety meeting with all involved and completing a job safety analysis form. Work began at 9:45 am with a ground and tower base inspection. We began climbing the tower at 10:00 am. The climbing inspection ended at approximately 11:15 am. All aerial maintenance issues were noted and photographed. TEP left the site at 2:00 pm.
Maintenance Accomplished:
- Minor corrosion was cleaned with hand tools and coated with an approved cold galvanizing compound.
- TEP adjusted tension, twist, and plumb in accordance with USCG specifications.

EXECUTIVE SUMMARY
Photograph Observations and Recommendations
A.4. Climbing facilities, platforms, catwalks – all secure
Observation:
All safety climb cables and safety climb hardware, including the inside of the safety climb assemblies, on the wooden support poles exhibit surface corrosion. (Photo “A”)
Recommendation:
Replace safety climb assemblies.
B.2. Rust and/or corrosion condition including mounts and accessories
Observation:
Surface corrosion was observed on hardware on all flanges.
Moderate corrosion was observed on hardware on the flange at elevation 40’ and on the collars of all (6) wooden support poles. TEP cleaned and galvanized areas where corrosion was present. (Photos “B”, “C”, “D”, and “E”)
Replace hardware affected by moderate corrosion.

Observation:
Corrosion and section loss were observed on the 5/8” diameter x 12” bent threaded rods on wood poles 1, 2, 5, and
6. (Photos “F”, “G”, and “H”)
Replace the threaded rods.

H.1. Strand condition (corrosion, breaks, nicks, kinks, etc.)
Observation:
Surface corrosion was observed on the guy wire strand connecting to the tower at elevation 40’. (Photo “J”)
Properly clean and galvanize the affected area.
H.2.a. Guy Hardware Conditions (Turnbuckles or equivalent (secure and safety properly applied)
Observation:
All safety wires in the guy wire turnbuckles are corroded and have only (1) wire rope clip installed. (Photo “K”)
Recommendation:
Remove the existing safety wire and wire rope clip and replace with new 3/8” EHS safety wire installed in a figure 8 configuration with (2) new wire rope clips.
H.2.a. Guy Hardware Conditions (Turnbuckles or equivalent (secure and safety properly applied)
Observation:
Outer guy anchor #6 has a bent 3/4” x 12” turnbuckle, not allowing for the turnbuckle to be adjusted. (Photo “L”)
Replace the damaged turnbuckle.
L
J
K
H.2.b. Guy Hardware Conditions (Cable thimbles properly in place (if required))
Observation:
Bent thimbles were observed at outside anchors 1 and 5.
(Photo “M”)
Recommendation:
Replace the bent thimbles.
H.2.d.ii. Guy Hardware Conditions (Cable connectors (end fittings) (Wire serving properly applied))
Observation:
No wire serving was observed at inside anchor 1 and outside anchors 2, 3, 5, and 8. (Photo “N”)
Recommendation:
Properly serve the ends of the guy wires.
H.3. Guy tensions
Observation:
The tension in outer guy wire 10 measures 1.84 kips, a value 8.7% out of range above the target tension value. The turnbuckle is fully extended, and tension cannot be lowered.
(Photo “O”)
Recommendation:
Install a preform that allows for the wire to be properly tensioned.
