General Comments:
Tower Engineering Professionals (TEP) completed a periodic inspection for the above referenced site. The onsite investigation was performed by Kelsey Spencer, CWI and Logan Hughes of TEP during the October 11, 2019 site visit. The inspection was in accordance with the Scope of Services for tower inspections, maintenance, and repairs of U.S. Coast Guard Towers dated January 2019, the U.S. Coast Guard Tower Manual CIM M11000.4A, FAA AC 70/7460-1L, TIA-322, ANSI/ASSE A10.48, and ANSI/TIA-222-G-2005, Annex J: Maintenance and Condition Assessment (Normative), including all addendums (addendums TIA-222-G-1 2007 and TIA 222-G-2 2009); the checklist is pages 4 thru 9 of this report.
Observations and recommendations are listed herein. For this inspection, the tower legs were named alphabetically according to the position of the ladder per the U.S. Coast Guard Tower Manual CIM M11000.4A.
The overall condition of the tower is poor with a few exceptions. This is detailed in the report. No unused equipment was identified on structure.
Chronological Narrative:
TEP arrived onsite at 8:45 am. We began by conducting a tail gate safety meeting with all involved and completing a job safety analysis form. Work began on this tower at 9:00 am with a ground and tower base inspection. We began climbing the tower at 9:30 am. The climbing inspection ended at approximately 11:45 am. All aerial maintenance issues were noted and photographed. TEP left the site at 12:45pm.
Maintenance Accomplished:
- Moderate to severe corrosion was observed and cleaned with hand tools and coated with an approved cold galvanizing compound.

EXECUTIVE SUMMARY
Photograph Observations and Recommendations
A.1. Damaged members (legs and bracing)
Observation:
Scaling was observed at the following locations:
B leg from 20’-30’ Several 2”x2”x1/4” angle members throughout the structure The majority of 5/8” diameter stitch bolt All flanges and flange hardware at elevations 20’, 40’, and 60’ All platform bolts at the top of the tower The majority of 5/8” diameter diagonal bolts
Recommendation:
A corrosion inspection should be performed to determine the extent of corrosion present on the structure.
A
C
B
A.4. Climbing facilities, platforms, catwalks – all secure
Observation:
Scaling was observed along the entirety of the right side of the ladder.
Replace the climbing ladder and all connection hardware.
B.1. Paint and/or galvanizing condition
Observation:
The finish is faded on the entire structure.
Recommendation:
Assess the tower finish during the inspection to determine if repainting of the tower is required.
D
F
E
B.2. Rust and/or corrosion condition including mounts and accessories
Observation:
Surface to severe corrosion was observed on the majority of the structure. TEP cleaned and galvanized corroded areas.
Recommendation:
A corrosion inspection should be performed to determine the extent of corrosion present on the structure.
I
G
H accessories (cont.)
accessories
Observation:
Severe corrosion was observed on the entirety of the wave guide.
Recommendation:
Replace the wave guide.
D.3. Lightning protection (secured to structure)
Observation:
The lightning rod is not the tallest appurtenance on the structure. An omni antenna is the tallest appurtenance on the tower.
Recommendation:
Install lightning rod so that it is the tallest appurtenance on the structure.
L
J
K
I.1.c. Ground condition (Site condition (standing water, drainage, trees, etc.))
Observation:
The compound gating failed in several locations. Severe corrosion is typical throughout the entirety of the compound fence.
Replace the compound fence.
I.1.c. Ground condition (Site condition (standing water, drainage, trees, etc.))
Observation:
Vegetative growth was observed inside the compound fence.
Recommendation:
Compound should be cleared of vegetative growth and sprayed to prevent future growth.
