PAST PERFORMANCE QUESTIONNAIRE
REQUEST FOR PROPOSALS-DETENTION SERVICES
I. INTRODUCTION:
This questionnaire is designed to collect past performance information on Offerors competing for award of a contract for Detention Services in California. Please complete the enclosed questionnaire and email it directly to CaliforniaDetention@ice.dhs.gov. This questionnaire can be duplicated as necessary. Handwritten responses will be accepted. If you need more space than provided, please attach additional pages or write on the last page. Please include only relevant information. Responses will be treated as source selection sensitive. Past Performance responses are requested by October 31, 2019 by 1200 EST

In Section V below of this questionnaire we ask questions and ask for your comments and ratings of the contractor’s performance. The following codes are associated with performance ratings:

| EXCEPTIONAL – Performance meets contractual requirements and exceeds many to the Government's benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective. |

| VERY GOOD – Performance meets contractual requirements and exceeds some to the Government's benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the Contractor were effective. |
| S |
| SATISFACTORY – Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the Contractor appear or were satisfactory. |
| M |
| MARGINAL – Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor's proposed actions appear only marginally effective or were not fully implemented. |
| U |
| UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the Contractor's corrective actions appear or were ineffective. |

| NOT APPLICABLE – Should be used if the ratings are not going to be applied to a particular area for evaluation. |
