About this file
This document is a Contractor Compliance Agreement form (DOC-2818) issued by the Wisconsin Department of Corrections Division of Management Services, dated April 2017. The form serves as a standardized compliance certification tool that contractors must complete and submit to the Department of Corrections Purchasing Services, Contract Manager, or Purchasing Helpdesk to affirm their adherence to contract requirements.
The agreement focuses on ensuring contractor compliance with all contract terms and conditions, particularly emphasizing background check requirements for all contractor staff and applicants providing services to the Wisconsin Department of Corrections. The form requires contractors to provide authorized representative information, list all current DOC contract identification numbers, and commit to annual compliance verification. Once completed, the form covers all of a contractor's DOC contracts for a twelve-month period, with contractors required to submit updated compliance forms annually or within 30 days of each contract year-end date. The document does not specify particular goods or services, pricing terms, or contract amounts, as it serves as a general compliance certification mechanism applicable across various DOC contractor relationships.
