About this file
This is a Past Performance Questionnaire (PPQ) template for evaluating contractor performance on the restoration of Historic Theater Building No. 35 at James H. Quillen VA Medical Center in Mountain Home, Tennessee (Solicitation Number 36C24926R0055, Project 621-24-103). The document serves as an evaluation tool to assess prospective contractors' past performance capabilities prior to contract award.
The PPQ consists of two sections: Section A is completed by the prospective contractor providing their company information, point of contact, assessor details, and authorization to release performance information to NCO 9 Network Contracting Activity in Nashville, Tennessee; Section B is completed by independent assessors knowledgeable of the contractor's performance. Assessors evaluate five key performance areas using a five-point rating scale (Outstanding, Good, Acceptable, Marginal, Unacceptable): (1) Management Effectiveness, including personnel experience and system documentation; (2) Quality of Service and Workmanship, addressing professional service delivery and specification achievement; (3) Timeliness/Adherence to Schedules; (4) Training/Technical Support; and (5) Customer Satisfaction, including cost control and invoice accuracy. The questionnaire requires detailed rationale for any Marginal or Unacceptable ratings and concludes with an overall contractor performance rating and assessor recommendation regarding future contract awards. Completed questionnaires must be submitted to the Contracting Officer (jeannette.morphew@va.gov) and Contract Specialist (lara.hampton@va.gov). If contractors cannot obtain PPQ responses, the government reserves the right to utilize CPARS (Contractor Performance Assessment Reporting System) data for past performance evaluation.
