About this file
This is a Data Item Description (DID) form that establishes the standard format and content requirements for test/inspection reports used in federal contracting. The document is designated DI-NDTI-80809B and was approved on January 24, 1997, with the Office of Primary Responsibility assigned to F/AFMC-DOP. It supersedes earlier versions DI-NDTI-80809A and DI-MISC-80653.
The DID applies to engineering (developmental), preliminary qualification, qualification, and acceptance testing activities and specifies that contractor format is acceptable provided the required content is organized for clear presentation and understanding. The test/inspection report must include a cover and title page with report date, report number, contractor information, contract details, test type identification, item identification, test dates, government activity information, and security classification data. The report structure requires a table of contents identifying all major sections, appendices, and illustrations; an introduction containing test/inspection objectives, complete item identification (nomenclature, stock number, model/part/serial numbers, item type, engineering changes, specifications, and manufacture date), and required inspection parameters with performance requirements and compliance limits; a summary of significant results, observations, conclusions, recommendations, identified deviations, and supporting tables or graphs; and complete reference documentation. The body of the report must contain test equipment identification (nomenclature, model, serial number, manufacturer, calibration status, and accuracy data), facility installation and physical set-up descriptions, complete testing/inspection procedures, recorded data, results and analysis with matrices comparing achieved results against objectives, conclusions distinguishing objective from subjective findings, and recommendations regarding item acceptability, additional testing needs, redesign requirements, and documentation changes. The report must include three certifications: authentication that testing was performed per applicable plans and procedures with true and accurate results signed by contractor personnel and government witnesses; authentication identifying previously validated requirements not tested with the validation method employed; and authentication of item pass/fail status. Appendices may include detailed test data, drawings, photographs, and associated test/inspection reports too voluminous for the main report body.
