QUALITY CONTROL
PART 1 GENERAL
1.01 SCOPE OF WORK
A. This Section specifies the requirements for the following work items:
1. CONTRACTOR Quality Control.
2. Quality Assurance performed by the COR.
1.02 DEFINITION
A. Contractor Quality Control (CQC)
1. CQC is the responsibility of CONTRACTOR, and consists of observations, testing, measurements, and documentation generated and/or performed by the CONTRACTOR to confirm that the final product delivered to the COR meets the requirements of the Drawings, Specifications, and Contract Documents.
2. CQC is the construction CONTRACTOR’s system to manage, control and document CONTRACTOR’s suppliers, and CONTRACTOR’s subcontractors and the subcontractors’ activities to comply with Contract requirements.
B. Quality Assurance (QA)
1. QA is the responsibility of the COR or their designee and consists of observations and documentation generated and/or performed by the COR to check the Work of CONTRACTOR, and to check CQC performed by CONTRACTOR is accurate and complete.
2. QA is the system by which the COR confirms the CQC is functioning and the specified and contractually required end product is realized.
1.03 SUBMITTALS
A. Administrative
1. CQC Plan: The CONTRACTOR shall furnish for review by the COR, not later than 28 days after receipt of notice to proceed, the CONTRACTOR CQC Plan proposed to implement the requirements of the Contract. The plan shall identify personnel, procedures, control, instructions, tests, records, and forms to be used. The CQC Plan shall include, as a minimum, the following to cover all construction operations, both onsite and offsite, including Work by subcontractors, fabricators, suppliers, and purchasing agents:
a. A description of the quality control organization, including a chart showing lines of authority and acknowledgment that the CQC staff shall implement the

quality control system for all aspects of the Work specified. The staff shall include a CQC Manager who shall report to the project superintendent. Include a CQC organizational chart.
b. The name, qualifications (in resume format), duties, responsibilities, and authorities of each person assigned a CQC function.
c. Names, locations, and accreditation of proposed laboratory facilities.
d. Procedures for scheduling, reviewing, certifying, and managing submittals, including those of subcontractors, offsite fabricators, suppliers, and purchasing agents.
e. Control, verification, and acceptance testing procedures for each specific test to include the test name, specification paragraph requiring test, feature of work to be tested, test frequency, and person responsible for each test.
f. Procedures for tracking construction deficiencies from identification through acceptable corrective action. These procedures shall establish verification that identified deficiencies have been corrected.
g. Reporting procedures, including proposed reporting formats. A list of the definable features of Work. A definable feature of Work is a task which is separate and distinct from other tasks, has separate control requirements, and may be identified by different trades or disciplines, or it may be Work by the same trade in a different environment. Although each section of the Specifications may generally be considered as a definable feature of Work, there are frequently more than one definable feature under a particular Specification section.
B. Quality Control
1. CQC testing results and reports shall be submitted within 24 hours of performing the test or inspection.
1.04 TESTING STANDARDS
A. Testing standards and requirements as described in individual Specification sections.
B. Other test requirements as described in individual Specification sections.
1.05 QUALIFICATIONS
A. The CQC Manager must have a minimum of five (5) years of experience of related CQC management experience on similar dam construction and large earthwork construction projects.
B. The Quality Control Testing Laboratory shall have at least five (5) years of experience as a materials-testing laboratory including the performance of testing comparable to that required for this Project. The person in charge of the Work for the Quality Control Testing Laboratory shall be a Professional Engineer.

PART 3 EXECUTION
3.01 CONTRACTOR QUALITY CONTROL (CQC)
A. Perform CQC activities at or above the minimum frequencies specified in the individual Specification sections, and as necessary to demonstrate that the quality of the Work is within specified requirements.
B. CQC testing shall be performed by a testing laboratory or agency independent of the CONTRACTOR, licensed or certified in accordance with laws and regulations and applicable state and Tribal statutes. In the event state license or certification is not required, testing laboratories or agencies shall meet following applicable requirements:
1. “Recommended Requirements for Independent Laboratory Qualification,” published by the American Council of Independent Laboratories.
2. Basic requirements of ASTM E329, “Standard of Recommended Practice for Inspection and Testing Agencies for Concrete and Steel as Used in Construction.”
3. Calibrate testing equipment at reasonable intervals by devices of accuracy traceable to either the National Bureau of Standards or accepted values of natural physical constants.
C. CQC responsibilities generally include, but may not be limited to:
1. Material property tests for all earthwork items.
2. Material property tests for reused on-site materials.
3. Tests on manufactured or fabricated items.
4. Field and laboratory quality control tests to control CONTRACTOR’s concrete material production operations.
5. Field and laboratory quality control tests to control CONTRACTOR’s earthwork operations.
6. Performing survey to confirm geometry.
D. Results of all Quality Control activities shall be made available to the COR at the CONTRACTOR’s site office by the end of the next working day.
3.02 QUALITY ASSURANCE (QA)
A. The COR and BIA Representative have the right to direct QC testing and to observe the work at any time. CONTRACTOR shall be responsible for cooperating with the COR and BIA Representative during all QC and QA testing activities and in resolving all problems

identified during testing. CONTRACTOR shall provide all equipment and labor required to safely access QC testing locations. CONTRACTOR shall repair any damage to finished Work caused by QC sampling or testing activities.
B. QA activities will not in any way relieve the CONTRACTOR of performing CQC activities required by the Contract Documents.
C. If requested, the CONTRACTOR shall provide representative samples of any material used for this Project for QA testing. The COR and the BIA Representative reserve the right to sample materials at the source of production.
D. Acceptance of the Work will be based on the COR’s review of the CONTRACTOR’s QC data, as well as on visual observation made by the QA team’s work, including any re-work effort required to complete the Project in compliance with the Contract Documents.
3.03 SUBSTANDARD WORK
A. The COR may order tests on substandard or damaged Work, equipment, or materials to determine the required functional capability for possible acceptance, if there is no other reason for rejection.
B. The cost of such tests shall be borne by CONTRACTOR and the nature, extent, and supervision of the tests will be as determined by the COR.
C. If the results of the tests indicate that the required functional capability of the Work, equipment, or material is impaired, consistent with the final general appearance of the same, the Work, equipment, or materials may be deemed substandard and shall be replaced by CONTRACTOR. CONTRACTOR may elect to replace the substandard Work or material in lieu of performing the tests.
3.04 REPORTING
A. Maintain current records providing factual evidence that required quality control activities and tests have been performed. Include in these records the Work of subcontractors and suppliers.
B. Submit CQC testing results or reports as required in the Drawings and Specifications. If no time frame is given for a specific test report, then submit within 24 hours of execution.
C. If testing results or reports are received in the specified time frames, then Work may be stopped at no additional expense to the COR.
