The project is divided into two work areas, each with a thirty calendar day completion requirement and a maximum total contract time of sixty consecutive calendar days from the Notice to Proceed, with liquidated damages assessed at $250 per calendar day for delays. Disadvantaged business enterprise (DBE) participation requirements apply to this federally-funded project, which receives funding through a Florida Department of Transportation (FDOT) grant combined with local share, with contract award contingent upon FDOT concurrence. Bidders must submit comprehensive documentation including vendor information forms, W-9 tax forms, proof of insurance compliance, sworn statements regarding public entity crimes, non-collusion affidavits, E-Verify registration confirmation, human trafficking attestations, conflict of interest certifications, drug-free workplace certifications, smoke-free workplace acknowledgements, active Sunbiz registration documentation, and SAM.gov registration listings with Unique Identifier Numbers.
